Overview of the medicine
Diastat-L (Loratadine) is a second-generation antihistamine used to relieve symptoms of allergic conditions such as hay fever and chronic urticaria. It effectively blocks histamine, reducing itching, sneezing, and watery eyes without causing significant drowsiness.
Uses & Indications
Dosage
Adults
10 mg once daily.
Elderly
No dosage adjustment generally required, but caution in severe hepatic or renal impairment.
Renal_impairment
For severe renal impairment (creatinine clearance <30 mL/min), 10 mg every other day.
How to Take
Oral administration. May be taken with or without food.
Mechanism of Action
Loratadine is a selective peripheral H1-receptor antagonist. It blocks the action of histamine, a substance produced by the body during an allergic reaction, thereby relieving allergic symptoms.

Excretion
Approximately 40% in urine and 42% in feces over 10 days, primarily as conjugated metabolites.
Half life
Approximately 8.4 hours (Loratadine); 28 hours (Desloratadine, active metabolite)
Absorption
Rapidly absorbed from the gastrointestinal tract. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Extensively metabolized in the liver to an active metabolite, desloratadine, primarily by CYP3A4 and CYP2D6.

Contraindications
- Hypersensitivity to Loratadine or any component of the formulation.
- Children under 2 years of age (syrup formulation might be used under medical supervision).

Cimetidine
May increase plasma concentrations of loratadine.
Erythromycin
May increase plasma concentrations of loratadine.
Ketoconazole
May increase plasma concentrations of loratadine.

Store in a cool, dry place, below 30°C. Protect from light and moisture.
Overdose
Symptoms of overdose may include somnolence, tachycardia, and headache. Treatment is symptomatic and supportive. Gastric lavage and activated charcoal may be considered.

Pregnancy Category B. Use only if clearly needed during pregnancy. Excreted in breast milk, so caution is advised in nursing mothers.
