Description
Overview of the medicine
DicloGel is a topical analgesic and anti-inflammatory gel used for the relief of pain, swelling, and inflammation associated with musculoskeletal conditions such as sprains, strains, arthritis, and lower back pain. It combines Diclofenac for its anti-inflammatory action with Methyl Salicylate, Menthol, and Linseed Oil for local pain relief and enhanced penetration.
Uses & Indications
Dosage
Adults
Apply a small amount (2-4g, equivalent to 2.5-5cm strip of gel) to the affected area 2 to 4 times daily, rubbing gently.
Elderly
No specific dose adjustment required; use as per adult dosage.
Renal_impairment
No specific dose adjustment required as systemic absorption is minimal.
How to Take
For external use only. Apply a thin layer of gel to the affected skin area and rub gently until it disappears. Wash hands thoroughly after application, unless the hands are the treated area.
Mechanism of Action
Diclofenac inhibits prostaglandin synthesis by blocking cyclooxygenase (COX) enzymes, thereby reducing inflammation and pain. Methyl Salicylate and Menthol act as counter-irritants, producing a cooling or warming sensation that distracts from pain. Linseed oil (Flaxseed oil) has anti-inflammatory properties and helps in the absorption of other active ingredients.
Pharmacokinetics
Onset
Rapid local pain relief, usually within 30 minutes to an hour.
Excretion
Primarily renal and biliary excretion for the systemically absorbed portion.
Half life
Systemic half-life of diclofenac is approximately 1-2 hours, but local therapeutic levels persist longer.
Absorption
Minimal systemic absorption after topical application. Approximately 6% of diclofenac from topical gel reaches systemic circulation.
Metabolism
The small amount absorbed systemically undergoes hepatic metabolism.
Side Effects
Contraindications
- Hypersensitivity to diclofenac, aspirin, other NSAIDs, or any component of the gel
- Application to open wounds, infected skin, or mucous membranes
- Third trimester of pregnancy
- Children below 12 years of age (consult a doctor)
Drug Interactions
0
Due to minimal systemic absorption, clinically significant drug interactions are unlikely with topical diclofenac. However, caution should be exercised if used concurrently with oral NSAIDs to avoid cumulative systemic exposure.
Storage
Store below 30°C in a cool, dry place, protected from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Systemic overdose with topical application is highly unlikely due to minimal absorption. Local overdose may lead to increased skin irritation. Management involves washing off excess gel and symptomatic treatment.
Pregnancy & Lactation
Pregnancy: Avoid in the third trimester due to potential fetal harm. Use with caution and only if clearly needed in the first and second trimesters after consulting a doctor. Lactation: It is not known whether diclofenac is excreted in human milk after topical application. Use with caution; do not apply to breast area.
