Description
Overview of the medicine
Esomeprazole is a proton pump inhibitor that reduces acid secretion in the stomach. It is used to treat conditions like GERD, ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
Varies based on indication. For GERD, 20-40 mg once daily for 4-8 weeks. For erosive esophagitis, 20-40 mg once daily for 4-8 weeks. For H. pylori eradication, 20 mg twice daily with antibiotics.
Elderly
No specific dosage adjustment required, but caution is advised.
Renal_impairment
No dosage adjustment needed for patients with mild to moderate renal impairment. For severe impairment, caution is advised.
How to Take
Take the capsule at least one hour before a meal, swallowed whole with water. Do not chew or crush.
Mechanism of Action
Esomeprazole irreversibly blocks the H+/K+-ATPase enzyme system (the proton pump) in gastric parietal cells, thereby inhibiting the final step in gastric acid secretion.
Pharmacokinetics
Onset
Within 1 hour.
Excretion
Primarily excreted in urine (80%) as metabolites, and the remainder in feces.
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is about 64% with a single 40 mg dose and increases to 89% with repeated once-daily administration. Food delays and decreases absorption but does not significantly affect efficacy.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, particularly by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.

Increased digoxin levels.
Warfarin
Increased INR and prothrombin time.
Clopidogrel
Reduced antiplatelet effect of clopidogrel due to CYP2C19 inhibition.
Methotrexate
