Overview of the medicine
Diloren-SR 100 mg Tablet is a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain, inflammation, and stiffness caused by conditions such as arthritis, ankylosing spondylitis, and acute musculoskeletal disorders. Its sustained-release formulation provides prolonged therapeutic effect.
Uses & Indications
Dosage
Adults
The usual recommended dose for adults is 100 mg once daily, preferably taken with food. The dose should be adjusted based on the patient's response and tolerability.
Elderly
Use with caution in elderly patients; a lower effective dose should be considered due to increased risk of side effects, especially GI bleeding and renal impairment.
Renal_impairment
Use with caution in patients with mild to moderate renal impairment. Dose adjustment may be necessary. Not recommended in severe renal impairment.
How to Take
Take the tablet whole with a glass of water, preferably with or after food to minimize gastrointestinal discomfort. Do not crush, chew, or break the tablet, as this may affect its sustained-release properties.
Mechanism of Action
Diclofenac Sodium acts by inhibiting cyclo-oxygenase (COX-1 and COX-2) enzymes, which are responsible for the synthesis of prostaglandins. Prostaglandins are mediators of inflammation, pain, and fever. By reducing prostaglandin production, Diclofenac alleviates these symptoms.
Pharmacokinetics
Onset
1-2 hours for significant pain relief, with sustained action over 24 hours.
Excretion
Approximately 60% of the dose is excreted in the urine as metabolites, and about 30-40% is excreted in the bile/faeces.
Half life
Plasma elimination half-life is approximately 1-2 hours, but the sustained-release mechanism extends its therapeutic effect significantly.
Absorption
Well absorbed from the gastrointestinal tract. Peak plasma concentrations are reached in 4-5 hours for SR formulations, providing prolonged drug release.
Metabolism
Undergoes extensive hepatic metabolism, primarily by CYP2C9 and to a lesser extent CYP2C19, forming several metabolites, some of which are active. Also undergoes glucuronidation.
Side Effects
Contraindications
- •Hypersensitivity to Diclofenac or any other NSAID, aspirin, or any component of the formulation.
- •Active gastrointestinal bleeding or peptic ulcer.
- •Severe renal or hepatic impairment.
- •Severe heart failure.
- •Third trimester of pregnancy.
- •History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.
Drug Interactions
Lithium and Methotrexate
May increase plasma levels of lithium and methotrexate, leading to toxicity.
Diuretics and ACE inhibitors
May reduce the natriuretic and antihypertensive effects of diuretics and ACE inhibitors, and increase the risk of renal impairment.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Other NSAIDs and Corticosteroids
Increased risk of gastrointestinal adverse effects.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, nausea, vomiting, epigastric pain, gastrointestinal bleeding, acute renal failure, and respiratory depression. Management involves symptomatic and supportive treatment. There is no specific antidote. Gastric lavage or activated charcoal may be considered if ingested recently.
Pregnancy & Lactation
Avoid use in the third trimester of pregnancy due to potential fetal harm (premature closure of ductus arteriosus). Use in early pregnancy and lactation only if clearly needed and the potential benefits outweigh the risks. Consult a doctor before use.
