Description
Overview of the medicine
Dimenta 5 mg Tablet contains Memantine, which is used to treat moderate to severe Alzheimer's disease. It helps improve memory, awareness, and the ability to perform daily functions by reducing abnormal activity in the brain.
Uses & Indications
Dosage
Adults
Initial: 5 mg once daily, gradually increased by 5 mg per week to a target dose of 10 mg twice daily (20 mg/day total).
Elderly
Same as adults, with careful titration.
Renal_impairment
Moderate (CrCl 30-49 mL/min): Max 10 mg/day. Severe (CrCl 5-29 mL/min): Max 5 mg/day.
How to Take
Administer orally, with or without food. Tablets can be taken whole.
Mechanism of Action
Memantine is a non-competitive N-methyl-D-aspartate (NMDA) receptor antagonist. It modulates the effects of glutamate, an excitatory neurotransmitter, preventing neuronal damage caused by excessive glutamate activity, while allowing normal physiological function.
Pharmacokinetics
Onset
Steady-state concentrations are reached within several days of dosing.
Excretion
Primarily renal, with approximately 48% excreted unchanged via urine. Excretion is pH dependent.
Half life
Approximately 60-80 hours.
Absorption
Well absorbed orally; absolute bioavailability of about 100%. Peak plasma concentration is reached in 3-7 hours.
Metabolism
Minimal hepatic metabolism. Approximately 48% of the administered dose is excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to memantine or any component of the formulation.
Drug Interactions
Sodium bicarbonate and other urine alkalinizers
May decrease memantine excretion, leading to increased plasma levels and potential toxicity.
NMDA antagonists (e.g., amantadine, ketamine, dextromethorphan)
Co-administration may increase the frequency and severity of CNS adverse reactions.
Drugs eliminated by renal tubular secretion (e.g., cimetidine, ranitidine, quinidine)
May alter plasma levels of memantine or the co-administered drug.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include agitation, confusion, hallucinations, seizures, stupor, coma, and slow heart rate. Management is supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category B. Use in pregnant women only if the potential benefit justifies the potential risk to the fetus. It is unknown whether memantine is excreted in human milk; use with caution in nursing mothers.
