Description
Overview of the medicine
Dipicard Plus is a combination medicine used to treat high blood pressure. It contains Carvedilol, a beta-blocker, and Hydrochlorothiazide, a diuretic, working together to relax blood vessels and remove excess fluid from the body.
Uses & Indications
Dosage
Adults
Usually 1 tablet once daily, adjusted based on response. Max 2 tablets daily.
Elderly
Initial dosage may be lower, titrated carefully.
Renal_impairment
Use with caution; dose adjustment may be necessary for severe impairment due to Hydrochlorothiazide.
How to Take
Take orally, with food to enhance absorption and reduce orthostatic effects of carvedilol. Take at the same time each day.
Mechanism of Action
Carvedilol is a non-selective beta-adrenergic blocker with alpha1-blocking activity. It reduces heart rate and myocardial contractility and causes vasodilation. Hydrochlorothiazide is a thiazide diuretic that inhibits sodium reabsorption in the distal convoluted tubules, leading to increased excretion of sodium, chloride, and water.

Excretion
Carvedilol is primarily excreted via bile/feces; Hydrochlorothiazide is primarily excreted renally.

6-10 hours (Carvedilol), 5.6-14.8 hours (Hydrochlorothiazide).
Absorption
Carvedilol is rapidly absorbed (approx. 25-35% bioavailability); Hydrochlorothiazide is variably absorbed (approx. 60-80%).
Metabolism
Carvedilol undergoes extensive hepatic metabolism (CYP2D6, 2C9); Hydrochlorothiazide is not metabolized.

Pregnancy & Lactation
Not recommended during pregnancy unless clearly necessary, due to potential fetal harm (HCTZ) and adverse effects on neonates (Carvedilol). Use with caution during lactation.
