Overview of the medicine
Dipred 0.05% Eye Drop is a corticosteroid used to treat inflammation and pain associated with ocular surgery and endogenous anterior uveitis. It works by reducing the body's inflammatory response.
Uses & Indications
Dosage
Adults
For post-operative inflammation and pain: One drop into the conjunctival sac of the affected eye(s) four times daily beginning 24 hours after surgery and continuing throughout the first 2 weeks post-surgery, followed by tapering as directed by physician. For anterior uveitis: One drop into the conjunctival sac of the affected eye(s) four times daily for 14 days, followed by tapering as directed by physician.
Elderly
No specific dosage adjustment is required for elderly patients.
Renal_impairment
No specific dosage adjustment is required for patients with renal impairment due to minimal systemic absorption.
How to Take
For ophthalmic use only. Shake well before use. Instill one drop into the conjunctival sac, being careful not to touch the dropper tip to the eye or any other surface to avoid contamination.
Mechanism of Action
Difluprednate, a corticosteroid, inhibits the inflammatory response to a variety of agents. It causes a decrease in inflammation by stabilizing lysosomal membranes, preventing release of destructive acid hydrolases from leukocytes; inhibiting macrophage accumulation in inflamed areas; reducing leukocyte adhesion to capillary endothelium; reducing dilation of capillaries and permeability of the vascular bed; decreasing extravasation of serum proteins; inhibiting kinin effects; decreasing prostaglandin synthesis; and reducing numbers of lymphocytes, eosinophils, phagocytic cells, and platelets.
Pharmacokinetics
Onset
Relatively rapid, usually within hours to a day for anti-inflammatory effects.
Excretion
Excreted mainly via urine as metabolites after systemic absorption.
Half life
The systemic half-life of difluprednate and its active metabolite is relatively short. Ocular tissue half-life is not well-defined but offers sustained local action.
Absorption
Minimal systemic absorption after topical ocular administration. Local ocular absorption is adequate for therapeutic effect.
Metabolism
Primarily metabolized in the eye to its active metabolite, difluprednisolone butyrate, and other inactive metabolites.
Side Effects
Contraindications
- •Acute untreated purulent bacterial, fungal, or viral infections of the eye (e.g., herpes simplex keratitis).
- •Mycobacterial infections of the eye.
- •Hypersensitivity to difluprednate or any components of the medication.
Drug Interactions
Topical NSAIDs
Concomitant use with topical NSAIDs may increase the risk of corneal healing problems.
Storage
Store upright at 15°C to 25°C (59°F to 77°F). Protect from freezing. Keep out of reach of children.
Overdose
Overdose is unlikely with ophthalmic administration. If accidentally ingested, supportive care and observation are recommended.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Caution should be exercised when administered to a nursing woman.
