Description
Overview of the medicine
Dismonal 60 mg tablet contains Tolperisone Hydrochloride, a centrally acting muscle relaxant used for symptomatic treatment of spasticity and muscle spasm conditions.
Uses & Indications
Dosage
Adults
Initially 50 mg to 150 mg three times daily, depending on individual need and tolerability. Dismonal 60 mg can be taken as one tablet three times daily, adjusted by physician.
Elderly
Similar to adult dosage, but caution advised; dosage adjustments may be necessary based on renal/hepatic function.
Renal_impairment
Mild to moderate impairment: Dosage reduction and careful monitoring. Severe impairment: Not recommended.
How to Take
Oral administration. Take with a glass of water, preferably after meals to reduce gastrointestinal discomfort.
Mechanism of Action
Tolperisone acts on the central nervous system, primarily by stabilizing neuronal membranes and inhibiting voltage-gated sodium channels. It also has a weak alpha-adrenergic blocking effect and inhibits calcium influx, reducing muscle tone without significant sedative effects.
Pharmacokinetics
Onset
Within 1-2 hours for muscle relaxation effects.
Excretion
Mainly excreted by the kidneys, primarily as metabolites.
Half life
Approximately 1.5-2.5 hours.
Absorption
Well absorbed from the gastrointestinal tract, but undergoes extensive first-pass metabolism, leading to low bioavailability (around 17%). Peak plasma concentrations are reached within 0.5-1.5 hours.
Metabolism
Extensively metabolized in the liver by cytochrome P450 enzymes (CYP2D6, CYP2B6, CYP1A2, CYP2C19, CYP3A4, CYP2C9) and other pathways, forming several metabolites, some of which are active.
Side Effects
Contraindications
- Hypersensitivity to Tolperisone or any component of the tablet
- Myasthenia gravis
- Children below 6 years
Drug Interactions
Methocarbamol
May increase the concentration of Tolperisone.
NSAIDs (e.g., Naproxen, Ibuprofen)
Concomitant use may enhance effects due to metabolic interactions (CYP2D6 inhibition by Tolperisone affecting some NSAIDs).
Other centrally acting muscle relaxants
Increased risk of sedation, although Tolperisone itself has minimal sedative effects.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include drowsiness, dizziness, abdominal pain, tachycardia. Management is symptomatic and supportive; no specific antidote exists.
Pregnancy & Lactation
Pregnancy: Use only if clearly necessary, as animal studies are inconclusive and human data are limited. Lactation: Not recommended, as it is unknown if Tolperisone is excreted in breast milk.
