Description
Overview of the medicine
Ditrolac is a non-steroidal anti-inflammatory drug (NSAID) containing Ketorolac Tromethamine. It is primarily used for the short-term management of moderate to severe acute pain. It works by reducing hormones that cause inflammation and pain in the body.
Uses & Indications
Dosage
Adults
10 mg every 4-6 hours as needed, not to exceed 40 mg/day. Treatment duration should not exceed 5 days.
Elderly
Lower doses may be required; dosage should be adjusted based on renal function.
Renal_impairment
Contraindicated in patients with moderate to severe renal impairment. For mild impairment, use reduced dose and monitor renal function.
How to Take
Oral tablets should be swallowed whole with water, preferably with food or milk to minimize gastrointestinal upset.
Mechanism of Action
Ketorolac inhibits prostaglandin synthesis by irreversibly inhibiting cyclooxygenase (COX-1 and COX-2) enzymes. This reduces the formation of prostaglandins, which are mediators of pain, inflammation, and fever.
Pharmacokinetics
Onset
Analgesic effect typically begins within 30 minutes after oral administration.
Excretion
Primarily renal (approximately 90% as metabolites and unchanged drug), with some fecal excretion.
Half life
Approximately 4-6 hours.
Absorption
Rapidly and completely absorbed orally. Peak plasma concentrations reached within 30-60 minutes after oral administration.
Metabolism
Primarily hepatic, mainly via hydroxylation and conjugation.
Side Effects
Contraindications
- Active peptic ulcer, gastrointestinal bleeding or perforation
- Severe renal impairment
- Hypersensitivity to ketorolac or other NSAIDs
- Patients with a history of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- Coronary Artery Bypass Graft (CABG) surgery
- Concomitant use with other NSAIDs, aspirin, or anticoagulants
- Pregnancy and lactation
Drug Interactions
Lithium
Increased plasma lithium levels, leading to toxicity.
Methotrexate
Increased methotrexate levels, leading to toxicity.
Corticosteroids
Increased risk of GI adverse events.
Warfarin/Anticoagulants
Increased risk of bleeding.
Diuretics/ACE Inhibitors
Reduced efficacy of diuretics and ACE inhibitors, potential for renal impairment.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include nausea, vomiting, epigastric pain, lethargy. Management involves symptomatic and supportive care, gastric lavage if recent ingestion, and close monitoring of renal function. Hemodialysis is not likely to be effective.
Pregnancy & Lactation
Contraindicated during pregnancy, especially in the third trimester due to risk of premature closure of the fetal ductus arteriosus and renal dysfunction. Avoid during breastfeeding as it is excreted in breast milk.
