Description
Overview of the medicine
Dobical-Plus 4/2 Rectal Ointment contains Calcium Dobesilate (a vasoprotective agent) and Lidocaine HCl (a local anesthetic) used for the symptomatic relief of pain, itching, and discomfort associated with internal and external hemorrhoids and anal fissures.
Uses & Indications
Dosage
Adults
Apply a small amount of ointment to the affected area 2-3 times daily, especially after bowel movements. For internal hemorrhoids, use the provided applicator.
Elderly
No specific dosage adjustment is generally required for elderly patients; use as for adults.
Renal_impairment
No specific dosage adjustment needed for topical application due to minimal systemic absorption.
How to Take
For external use, apply directly to the perianal area. For internal hemorrhoids, attach the applicator to the tube and insert it gently into the rectum, squeezing the tube to release the ointment. Wash hands thoroughly before and after use. Avoid contact with eyes and mucous membranes.
Mechanism of Action
Calcium Dobesilate acts by improving capillary resistance and reducing capillary permeability, thereby minimizing bleeding and exudation. Lidocaine HCl provides local anesthesia by blocking nerve impulse conduction, rapidly relieving pain, itching, and discomfort in the affected area.
Pharmacokinetics
Onset
Lidocaine's local anesthetic effect begins rapidly, typically within 2-5 minutes after application.
Excretion
Lidocaine metabolites and a small percentage of unchanged Lidocaine are excreted by the kidneys. Calcium Dobesilate is primarily excreted unchanged via the kidneys.
Half life
Lidocaine's plasma half-life is approximately 1.5-2 hours in healthy adults.
Absorption
Calcium Dobesilate: Poorly absorbed orally; minimal systemic absorption following topical application. Lidocaine: Readily absorbed from mucous membranes; systemic absorption can occur with topical application, especially on damaged skin.
Metabolism
Lidocaine is extensively metabolized in the liver by microsomal enzymes, primarily to monoethylglycinexylidide (MEGX) and then to glycinexylidide (GX). Calcium Dobesilate is largely eliminated unchanged.
Side Effects
Contraindications
- Known hypersensitivity to Calcium Dobesilate, Lidocaine, or any other component of the ointment.
- Children under 12 years of age without medical advice.
Drug Interactions
Systemic Local Anesthetics
Although systemic absorption is minimal, concurrent use of large amounts of other local anesthetics may increase the risk of systemic toxicity.
Storage
Store below 30°C in a dry place, protected from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Systemic overdose is highly unlikely with proper topical application. If accidentally ingested, or if excessive amounts are applied to large, broken skin areas, seek immediate medical attention. Symptoms of Lidocaine overdose may include CNS effects (dizziness, drowsiness, seizures) and cardiovascular effects (bradycardia, hypotension).
Pregnancy & Lactation
Use during pregnancy and lactation should be undertaken only after consulting a doctor and assessing the potential benefits against risks. Limited data are available.
