Description
Overview of the medicine
Dolin (Paracetamol) is an analgesic and antipyretic drug used to relieve pain and reduce fever. It is one of the most commonly used over-the-counter medications.
Uses & Indications
Dosage
Adults
Oral: 500-1000 mg every 4-6 hours as needed. Maximum 4g (4000 mg) in 24 hours.
Elderly
Generally similar to adult dosage, but caution should be exercised in elderly patients with impaired liver or renal function.
Renal_impairment
Dose interval may need to be extended in patients with severe renal impairment (e.g., every 8 hours for CrCl < 10 mL/min).
How to Take
Oral administration, usually with water. Can be taken with or without food.
Mechanism of Action
Paracetamol's mechanism of action is not fully understood, but it is believed to involve inhibition of prostaglandin synthesis, primarily within the central nervous system, leading to its analgesic and antipyretic effects.
Pharmacokinetics
Onset
Onset of action for pain relief is typically within 30-60 minutes.
Excretion
Excreted primarily in the urine as glucuronide and sulfate conjugates, with less than 5% excreted as unchanged drug.
Half life
Plasma half-life is typically 1 to 4 hours.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract following oral administration.
Metabolism
Extensively metabolized in the liver, primarily by conjugation with glucuronic acid and sulfate. A small amount is metabolized by cytochrome P450 to a reactive intermediate (NAPQI), which is normally detoxified by glutathione.
Side Effects
Contraindications
- Known hypersensitivity to paracetamol
- Severe hepatic impairment
Drug Interactions
Alcohol
Increased risk of hepatotoxicity with chronic alcohol consumption
Warfarin
May increase anticoagulant effect with prolonged daily use
Certain anticonvulsants (e.g., phenobarbital, phenytoin, carbamazepine)
May increase paracetamol metabolism, potentially reducing efficacy and increasing risk of hepatotoxicity
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose can cause severe, potentially fatal hepatotoxicity. Symptoms may include nausea, vomiting, abdominal pain. Treatment involves administration of N-acetylcysteine (NAC) as an antidote.
Pregnancy & Lactation
Considered generally safe for use during pregnancy and lactation at recommended therapeutic doses, but always consult a healthcare professional.

- Advise patients not to exceed 4g/day (or lower in some cases like chronic alcoholics).
- Caution in patients with hepatic or severe renal impairment.
- Counsel patients on identifying paracetamol in combination products to avoid accidental overdose.
Patient Guidelines
- Do not exceed the recommended dose as it can cause serious liver damage.
- Do not take with other paracetamol-containing products.
- Consult a doctor if symptoms persist or worsen.
Missed Dose Advice
If a dose is missed, take it as soon as you remember, unless it is almost time for your next dose. Do not double the dose to catch up.
