Doltro 10 mg Tablet contains Dolutegravir, an antiretroviral medicine used in combination with other antiretroviral agents for the treatment of Human Immunodeficiency Virus type 1 (HIV-1) infection.
Uses & Indications
Dosage
Adults
Not typically used for adults as a single agent at this strength. For specific pediatric dosing or combination therapy, consult physician.
Elderly
No specific dose adjustment needed based on age alone. If used, follow adult guidelines with caution.
Renal_impairment
No dose adjustment required for mild, moderate, or severe renal impairment.
Children (15 kg to <20 kg)
10 mg once daily.
How to Take
Oral administration. Can be taken with or without food. Should be swallowed whole with water.
Mechanism of Action
Dolutegravir inhibits HIV integrase by binding to the integrase active site and blocking the strand transfer step of retroviral deoxyribonucleic acid (DNA) integration which is essential for the HIV replication cycle.

Antiviral effects typically observed within days of initiation.
Excretion
Mainly excreted in feces (approximately 53%) with a small amount excreted in urine (approximately 32% as metabolites, <1% as unchanged drug).
Half life
Approximately 14 hours (terminal half-life).
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations typically reached within 2-3 hours. Bioavailability is approximately 80%.
Metabolism
Primarily metabolized by UGT1A1 with a minor contribution from CYP3A. Renal excretion is a minor pathway.
Side Effects
Contraindications

Drug Interactions
Metformin
Dolutegravir can increase metformin concentrations; consider dose reduction of metformin.
Dofetilide
Increased dofetilide plasma concentrations, potentially leading to life-threatening arrhythmias. Contraindicated.
Rifampicin
Reduced dolutegravir plasma concentrations; dose adjustment of dolutegravir may be required.
Antacids (containing polyvalent cations)
Reduced dolutegravir absorption; administer dolutegravir at least 2 hours before or 6 hours after antacids.
Storage
Store below 30°C in a dry place, away from direct sunlight. Keep out of reach of children.
Overdose
Limited experience with dolutegravir overdose. Management should consist of general supportive measures, including monitoring of vital signs and observation of the clinical status of the patient. Dialysis is unlikely to be beneficial.
Pregnancy & Lactation
Pregnancy Category B. Studies have shown no increased risk of neural tube defects when dolutegravir is initiated during pregnancy. Consult a doctor for use during pregnancy. Not recommended for breastfeeding due to potential for HIV transmission and adverse effects.
