Description
Overview of the medicine
Domperidone is an antiemetic and prokinetic drug. It is used to relieve nausea, vomiting, and certain stomach problems like dyspepsia and gastroparesis.
Uses & Indications
Dosage
Adults
10 mg three times daily, preferably 15-30 minutes before meals. Maximum recommended daily dose is 30 mg.
Elderly
Similar to adults, but caution advised due to potential cardiac risks. Lower dose may be considered.
Renal_impairment
Dose reduction necessary; 10 mg once or twice daily depending on severity. Avoid in severe renal impairment (CrCl < 30 mL/min).
How to Take
Take orally, preferably 15-30 minutes before meals. Do not chew, crush, or break the tablet.
Mechanism of Action
Blocks dopamine D2 receptors primarily in the chemoreceptor trigger zone (CTZ) and peripherally in the gastrointestinal tract, leading to antiemetic and prokinetic effects.

Side Effects
Contraindications
- Known hypersensitivity to domperidone or any excipients.
- Patients with prolactin-releasing pituitary tumor (prolactinoma).
- Moderate to severe hepatic impairment.
- Concomitant use with strong CYP3A4 inhibitors (e.g., ketoconazole, erythromycin).
- Patients with cardiac conduction prolongation (e.g., QTc prolongation), significant electrolyte disturbances or underlying cardiac diseases (e.g., congestive heart failure).
- Patients with gastrointestinal haemorrhage, mechanical obstruction, or perforation.
Drug Interactions
Dopamine agonists (e.g., bromocriptine, cabergoline)
Domperidone may antagonize their effects.
Antacids/Antisecretory agents (e.g., proton pump inhibitors)
May reduce oral bioavailability of domperidone. Administer domperidone before meals and these agents after meals.
Strong CYP3A4 inhibitors (e.g., ketoconazole, erythromycin, clarithromycin)
Increase domperidone plasma levels, significantly increasing cardiac risk (QT prolongation). Concomitant use is contraindicated.
QT-prolonging drugs (e.g., some antiarrhythmics, antipsychotics, certain antibiotics)
Add a risk of QT prolongation. Concomitant use is contraindicated.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include somnolence, disorientation, extrapyramidal reactions, and potentially cardiac arrhythmias. Treatment is symptomatic and supportive. Gastric lavage and activated charcoal may be considered within the first hour of ingestion.
Pregnancy & Lactation
Use during pregnancy only if clearly needed and potential benefits outweigh risks, after consulting a doctor. Avoid during lactation as domperidone is excreted in breast milk and may cause adverse effects in the infant.
