Overview of the medicine
Domperidone 10 mg Tablet is a prokinetic and antiemetic agent used to treat nausea, vomiting, and certain gastrointestinal motility disorders. It works by blocking dopamine receptors in the brain and gut, thereby increasing the movement of food through the digestive system.
Uses & Indications
Dosage
Adults
10 mg up to three times daily, 15-30 minutes before meals. Maximum daily dose 30 mg.
Elderly
Lower doses may be considered. Individualize dosage based on response and tolerability. Maximum daily dose 30 mg.
Renal_impairment
In severe renal impairment (creatinine clearance < 30 ml/min), frequency should be reduced to once or twice daily, depending on severity, and dosage should be reduced.
How to Take
Take orally, preferably 15-30 minutes before meals. Do not crush or chew the tablet; swallow whole with water.
Mechanism of Action
Domperidone is a peripheral dopamine D2 receptor antagonist that primarily acts at the chemoreceptor trigger zone (CTZ) in the brain and in the gastrointestinal tract. By blocking dopamine receptors, it increases lower esophageal sphincter tone, improves gastroduodenal motility and peristalsis, and prevents reflux, leading to antiemetic and prokinetic effects.

Absorption

Metabolism
Extensively metabolized in the liver by cytochrome P450 (CYP3A4) hydroxylation and N-dealkylation.
Side Effects
Contraindications
- •Hypersensitivity to domperidone or any excipients
- •Prolactin-releasing pituitary tumour (prolactinoma)
- •Moderate to severe hepatic impairment
- •Conditions where stimulation of gastric motility could be harmful (e.g., gastrointestinal haemorrhage, mechanical obstruction, perforation)
- •Concomitant use of strong CYP3A4 inhibitors (e.g., ketoconazole, erythromycin)
Drug Interactions
Antacids and antisecretory agents
May reduce the oral bioavailability of domperidone. Administer domperidone before meals and antacids/antisecretory agents after meals.
Dopamine agonists (e.g., bromocriptine, cabergoline)
Domperidone may antagonize their effects.
QT-prolonging drugs (e.g., certain antipsychotics, antidepressants, antiarrhythmics)
Increased risk of QT prolongation. Avoid concomitant use.
Strong CYP3A4 inhibitors (e.g., ketoconazole, erythromycin, clarithromycin, fluconazole, amiodarone, diltiazem, verapamil)
Increased domperidone plasma levels, increased risk of QT prolongation and cardiac adverse events. Concomitant use is contraindicated.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include drowsiness, disorientation, extrapyramidal reactions (especially in children), and cardiac arrhythmias. Treatment is symptomatic and supportive, including gastric lavage and activated charcoal. ECG monitoring is recommended due to the risk of QT prolongation.
Pregnancy & Lactation
Pregnancy: Use with caution during pregnancy only if clearly needed and the potential benefits outweigh the potential risks to the fetus. Limited human data suggest no increased risk of major congenital malformations. Lactation: Domperidone is excreted in breast milk in small amounts. Monitor breastfed infants for potential cardiac effects. Consult a doctor.
