Description
Overview of the medicine
Donor 6 mg Tablet contains Donepezil Hydrochloride, a reversible acetylcholinesterase inhibitor, used for the symptomatic treatment of mild to moderately severe Alzheimer's disease. It helps improve cognitive functions like memory and thinking.
Uses & Indications
Dosage
Adults
Initial dose 5 mg once daily, taken in the evening. After 4-6 weeks, may increase to 10 mg once daily. For some patients, 23 mg once daily may be considered after at least 3 months on 10 mg/day (for certain extended-release formulations).
Elderly
No specific dose adjustment required based on age alone, but monitor for side effects and comorbidities.
Renal_impairment
No dose adjustment required for mild to moderate renal impairment. Use with caution in severe renal impairment.
How to Take
Take orally once daily in the evening, with or without food. Swallow the tablet whole with water.
Mechanism of Action
Donepezil selectively and reversibly inhibits acetylcholinesterase, the enzyme that hydrolyzes acetylcholine. This inhibition leads to increased concentrations of acetylcholine in the synaptic cleft, thereby enhancing cholinergic function in the central nervous system, which is believed to be deficient in Alzheimer's disease.
Pharmacokinetics
Onset
Therapeutic effects may take several weeks to manifest, with optimal benefits observed after months of consistent use.
Excretion
Excreted primarily in urine (57%) and feces (14%) as intact drug and metabolites.
Half life
Approximately 70 hours.
Absorption
Well absorbed from the gastrointestinal tract, reaching peak plasma concentrations in 3-4 hours. Bioavailability is 100%.
Metabolism
Extensively metabolized in the liver by cytochrome P450 enzymes (primarily CYP2D6 and CYP3A4) to several inactive and active metabolites.
Side Effects
Contraindications
- •Hypersensitivity to donepezil, piperidine derivatives, or any excipients present in the formulation.
Drug Interactions
Anticholinergics
May have antagonistic effects, reducing the efficacy of both drugs.
Succinylcholine, other neuromuscular blockers
Donepezil may exaggerate neuromuscular blockade.
Rifampicin, Phenytoin, Carbamazepine, Dexamethasone
May decrease donepezil levels by inducing CYP3A4 or CYP2D6, potentially reducing efficacy.
Ketoconazole, Quinidine, Fluoxetine, Cimetidine, Erythromycin
May increase donepezil levels by inhibiting CYP3A4 or CYP2D6, leading to increased risk of side effects.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe nausea, vomiting, salivation, sweating, bradycardia, hypotension, respiratory depression, collapse, convulsions, and increased muscle weakness. Management involves general supportive measures and atropine.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs the potential risk to the fetus. Donepezil is excreted in breast milk; therefore, avoid during lactation.
