Doribac 500 mg Capsule is a broad-spectrum antibiotic belonging to the carbapenem class, used to treat various severe bacterial infections. It is effective against both Gram-positive and Gram-negative bacteria, including many resistant strains.
Uses & Indications
Dosage
Adults
500 mg orally every 8 hours, for 5 to 14 days, depending on the infection severity and type. Dosage should be adjusted based on clinical response and microbiological results.
Elderly
No specific dosage adjustment is required for elderly patients with normal renal function. For elderly patients with renal impairment, dosage adjustment as per renal impairment guidelines is necessary.
Renal_impairment
For creatinine clearance (CrCl) 30 to 50 mL/min: 250 mg every 8 hours. For CrCl 10 to <30 mL/min: 250 mg every 12 hours. For CrCl <10 mL/min or hemodialysis: 250 mg every 24 hours (after dialysis on dialysis days).
How to Take
Take the capsule orally, with or without food. Swallow the capsule whole with a glass of water. Do not crush, chew, or open the capsule.
Mechanism of Action
Doripenem, the active ingredient in Doribac, works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located in the bacterial cell wall, which are essential for peptidoglycan synthesis, thereby leading to cell lysis and bacterial death.

Therapeutic concentrations typically achieved within 1-2 hours after administration.
Excretion
Primarily excreted unchanged in the urine (around 70% of the dose) via glomerular filtration and tubular secretion. Remaining portion excreted via non-renal routes.

Absorption
Rapidly absorbed from the gastrointestinal tract following oral administration. Bioavailability is high.
Metabolism
Minimal hepatic metabolism; primarily metabolized by dehydropeptidase-I (DHP-I) in the kidneys, but Doripenem is relatively stable to DHP-I.
Side Effects
Contraindications
- Known hypersensitivity to Doripenem, other carbapenems, or any component of the formulation.
- History of severe hypersensitivity reaction to any other beta-lactam antibacterial agent (e.g., penicillins, cephalosporins).
Drug Interactions
Probenecid
Probenecid competes with Doripenem for active tubular secretion, resulting in increased plasma concentrations and prolonged half-life of Doripenem. Co-administration is generally not recommended.
Oral Anticoagulants
Monitoring of INR is recommended during co-administration with warfarin as changes in anticoagulant effects have been reported with other beta-lactam antibiotics.
Valproic Acid/Divaproex
Carbapenems, including Doripenem, may decrease serum valproic acid concentrations to subtherapeutic levels, increasing the risk of seizures. Co-administration is generally not recommended.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In the event of an overdose, symptomatic and supportive treatment should be initiated. Doripenem can be removed by hemodialysis. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Doripenem is excreted in human milk; caution should be exercised when administered to a nursing woman.
