Doriject (Doripenem) is a broad-spectrum carbapenem antibiotic used to treat serious bacterial infections, including complicated intra-abdominal infections, complicated urinary tract infections, and pneumonia.
Uses & Indications
Dosage
Adults
500 mg administered by intravenous infusion over 1 hour every 8 hours. Duration of treatment varies based on infection type and severity (e.g., 5-14 days).
Elderly
No specific dose adjustment is needed based on age alone, but renal function should be considered.
Renal_impairment
Dose adjustment required for patients with creatinine clearance ≤ 50 mL/min (e.g., 250 mg every 8 or 12 hours).
How to Take
Administer intravenously via infusion over 1 hour. Do not administer as a bolus injection.
Mechanism of Action

Drug Interactions
Probenecid
Concomitant use of probenecid with doripenem is not recommended as it inhibits the renal tubular secretion of doripenem, increasing its plasma concentration and half-life.
Valproic acid
Concomitant use with carbapenems, including doripenem, may decrease valproic acid plasma concentrations to subtherapeutic levels. Monitor valproic acid levels and consider alternative anticonvulsant therapy.
Storage
Store dry powder at 20-25°C (68-77°F). Protect from light. Reconstituted solution should be used immediately or stored for a limited time under specific conditions.

In the event of an overdose, discontinue doripenem and institute general supportive treatment. Hemodialysis may be useful in enhancing the elimination of doripenem from the body.
