Overview of the medicine
Dos (doxycycline) is a broad-spectrum tetracycline antibiotic used to treat various bacterial infections, including respiratory tract infections, urinary tract infections, skin infections, and certain sexually transmitted infections. It is also used for malaria prophylaxis and severe acne.
Uses & Indications
Dosage
Adults
Standard dose is 100 mg orally once or twice daily, depending on the infection. For severe infections, 100 mg every 12 hours.
Elderly
No specific dose adjustment is generally required, but caution is advised due to potential comorbidities.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment, as doxycycline is primarily excreted non-renally.
How to Take
Take orally with a full glass of water. It is best taken with food to minimize gastric irritation, but avoid dairy products, calcium supplements, iron preparations, and antacids within 2 hours of taking Dos. Remain upright for at least 30 minutes after taking the medicine to prevent esophageal irritation.
Mechanism of Action
Doxycycline inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit, thereby preventing the attachment of aminoacyl-tRNA to the ribosomal A site. This leads to the inhibition of bacterial growth and replication (bacteriostatic effect).

Excreted mainly unchanged in feces (via bile) and urine.
Half life
16-24 hours.
Absorption
Well absorbed after oral administration, approximately 90-100% absorption. Absorption is slightly reduced by food and dairy products.
Metabolism
Minimally metabolized; primarily inactivated by chelation.
Side Effects
Contraindications

Drug Interactions
Penicillins
Tetracyclines may interfere with the bactericidal action of penicillins.
Methotrexate
Increased risk of methotrexate toxicity.
Oral contraceptives
May reduce efficacy of oral contraceptives.
Anticoagulants (e.g., Warfarin)
May potentiate anticoagulant effects, requiring dose adjustment.
Antacids, iron preparations, calcium supplements
Decreased absorption of doxycycline. Administer 2 hours apart.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe nausea, vomiting, and diarrhea. Management is primarily symptomatic and supportive, including gastric lavage and appropriate fluid and electrolyte replacement. Hemodialysis is not effective in removing doxycycline.
Pregnancy & Lactation
Pregnancy Category D. Doxycycline crosses the placenta and can cause permanent tooth discoloration and skeletal abnormalities in the fetus. Generally contraindicated in pregnant and lactating women. Consult a doctor.
