Overview of the medicine
Doxacil 100 mg capsule contains Doxycycline, a broad-spectrum tetracycline antibiotic used to treat various bacterial infections including respiratory tract infections, urinary tract infections, skin infections, and certain sexually transmitted diseases.
Uses & Indications
Dosage
Adults
Typical dose for most acute infections is 100 mg every 12 hours on the first day, followed by 100 mg once daily or 50 mg twice daily. For severe infections, 100 mg every 12 hours throughout the course. For malaria prophylaxis: 100 mg once daily, starting 1-2 days before travel and continuing for 4 weeks after returning.
Elderly
No specific dose adjustment required, but caution advised with hepatic impairment.
Renal_impairment
No dose adjustment necessary for renal impairment as it is primarily excreted non-renally.
How to Take
Take the capsule with a full glass of water, preferably with food, to reduce esophageal irritation. Avoid lying down for at least 30 minutes after taking. Do not break or crush the capsule.
Mechanism of Action
Doxycycline inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit, thereby preventing the attachment of aminoacyl tRNA to the ribosomal A site. This action is bacteriostatic.

Onset
Clinical effects typically observed within hours to days, depending on the infection. Bacteriostatic effect begins shortly after administration.
Excretion
Primarily excreted unchanged via fecal route (bile) and to a lesser extent via urine. Does not accumulate significantly in renal failure.
Half life
16-22 hours (average 18 hours).
Absorption
Well absorbed orally, approximately 90-100%. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Partially metabolized in the liver to inactive metabolites. About 30-60% undergoes enterohepatic recirculation.
Side Effects
Contraindications

Drug Interactions
Warfarin
May enhance anticoagulant effect. Monitor INR.
Oral contraceptives
May reduce the efficacy of oral contraceptives. Consider alternative contraception.
Barbiturates, carbamazepine, phenytoin
May decrease doxycycline half-life, requiring increased doxycycline dosage.
Iron preparations, bismuth subsalicylate
Reduce doxycycline absorption. Separate administration by 2-3 hours.
Antacids containing aluminum, calcium, or magnesium
Reduce doxycycline absorption. Separate administration by 2-3 hours.
Storage
Store in a cool, dry place, away from light and moisture, below 30°C. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, diarrhea, and potentially liver or kidney damage. Benign intracranial hypertension has been reported. Treatment is symptomatic and supportive, including gastric lavage and activated charcoal if recent ingestion.
Pregnancy & Lactation
Category D in pregnancy (due to potential for permanent tooth discoloration and skeletal retardation in the fetus if exposed during tooth and bone development). Not recommended during lactation as it is excreted in breast milk and can cause similar effects in nursing infants.
