Description
Overview of the medicine
Doxycycline is a broad-spectrum tetracycline antibiotic used to treat various bacterial infections, malaria prophylaxis, and certain sexually transmitted infections. It works by inhibiting bacterial protein synthesis.
Uses & Indications
Dosage
Adults
Typical adult dosage is 100 mg orally once or twice daily depending on the infection. For severe infections, 100 mg every 12 hours. For malaria prophylaxis: 100 mg once daily.
Elderly
No specific dosage adjustment needed for elderly unless severe hepatic impairment.
Renal_impairment
No dosage adjustment necessary for renal impairment, as it is primarily excreted non-renally.
How to Take
Take capsules orally with a full glass of water, preferably while sitting or standing, to reduce the risk of esophageal irritation. Can be taken with food (but not dairy) to minimize gastric upset.
Mechanism of Action
Doxycycline inhibits bacterial protein synthesis by reversibly binding to the 30S ribosomal subunit, thereby preventing the attachment of aminoacyl tRNA to the acceptor site on the mRNA-ribosome complex.
Pharmacokinetics
Onset
Antibacterial effects usually within 24-48 hours.
Excretion
Primarily excreted unchanged in faeces via intestinal secretion (non-biliary) and to a lesser extent in urine.
Half life
16-22 hours (can be longer in renal impairment).
Absorption
Well absorbed orally (approx. 90-100%), even with food, though dairy products and antacids can reduce absorption.
Metabolism
Minimally metabolized; primarily inactivated by chelate formation in the gut.
Side Effects
Contraindications
- Hypersensitivity to doxycycline or other tetracyclines
- Pregnancy (especially second/third trimester)
- Children under 8 years of age (due to potential for tooth discoloration and bone growth inhibition)
- Severe hepatic impairment
Drug Interactions
Warfarin
May enhance anticoagulant effect; monitor INR.
Penicillins
May interfere with the bactericidal action of penicillins; avoid co-administration.
Oral contraceptives
May reduce efficacy of hormonal contraceptives; advise backup birth control.
Barbiturates, carbamazepine, phenytoin
Can decrease doxycycline half-life through enzyme induction.
Antacids, iron supplements, calcium supplements, bismuth subsalicylate
Can decrease doxycycline absorption. Separate administration by at least 2-3 hours.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms include nausea, vomiting, diarrhea, and potentially liver toxicity. Treatment is supportive; gastric lavage or induction of emesis may be considered soon after ingestion. Hemodialysis is not effective.
Pregnancy & Lactation
Pregnancy Category D. Contraindicated during pregnancy (especially in the second and third trimesters) due to potential for permanent tooth discoloration and skeletal effects in the fetus. Not recommended during lactation as it is excreted in breast milk and may affect bone and tooth development in infants.
