Overview of the medicine
Doxicap 50 mg capsule contains Doxycycline, a tetracycline antibiotic used to treat various bacterial infections including respiratory tract infections, urinary tract infections, skin infections, and certain sexually transmitted diseases.
Uses & Indications
Dosage
Adults
General dosage: 100 mg once daily or 50 mg twice daily. For severe infections, 100 mg twice daily. For acne: 50 mg once daily.
Elderly
No specific dose adjustment is generally required, but caution is advised in patients with significant hepatic impairment.
Renal_impairment
No dose adjustment is necessary in patients with renal impairment, as it is primarily excreted non-renally.
How to Take
Take orally with a full glass of water, either with food or milk to reduce gastrointestinal upset. Avoid lying down for at least 30 minutes after taking the capsule to prevent esophageal irritation.
Mechanism of Action
Doxycycline inhibits bacterial protein synthesis by reversibly binding to the 30S ribosomal subunit of susceptible bacteria, thereby preventing the addition of amino acids to the growing peptide chain.

Therapeutic levels achieved within 1-2 hours after oral administration.
Excretion
Primarily excreted in the feces via bile and intestinal secretions, with a minor portion excreted renally.
Half life
Plasma half-life is approximately 16-22 hours, allowing for once or twice daily dosing.

Well absorbed orally (90-100%), even in the presence of food, though dairy products and antacids can reduce absorption.

Contraindications
- Hypersensitivity to doxycycline or any other tetracycline antibiotics.
- Pregnancy (especially in the second and third trimesters) due to potential tooth discoloration and skeletal development issues in the fetus.
- Children under 8 years of age due to risk of permanent tooth discoloration and enamel hypoplasia.
Drug Interactions
Warfarin
May enhance anticoagulant effects; monitor INR.
Penicillins
May interfere with the bactericidal action of penicillins; avoid concomitant use.
Oral contraceptives
May decrease efficacy of oral contraceptives; use alternative birth control.
Barbiturates, Carbamazepine, Phenytoin
May decrease doxycycline's half-life.
Antacids, iron supplements, calcium-containing products
Can decrease doxycycline absorption. Administer at least 2 hours before or 4 hours after doxycycline.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

Symptoms of overdose may include severe gastrointestinal disturbances (nausea, vomiting, diarrhea). Management involves symptomatic and supportive care, including gastric lavage or induced emesis if appropriate. Doxycycline is not readily removed by hemodialysis.
Pregnancy & Lactation
Pregnancy Category D: Doxycycline can cause permanent tooth discoloration and skeletal developmental issues in the fetus. Not recommended during pregnancy, especially from the fourth month onwards. Lactation: Doxycycline is excreted in breast milk and may cause adverse effects in nursing infants; therefore, it is generally not recommended during breastfeeding.
