Description
Overview of the medicine
Doxycycline is a tetracycline antibiotic used to treat various bacterial infections, including respiratory tract infections, urinary tract infections, skin infections, acne, and certain sexually transmitted infections.
Uses & Indications
Dosage
Adults
Typically 100 mg orally once or twice daily, depending on the infection. For severe infections, 100 mg every 12 hours.
Elderly
No specific dose adjustment needed based on age, but caution in patients with hepatic impairment.
Renal_impairment
No dosage adjustment necessary as renal excretion is not the primary route.
How to Take
Take orally with a full glass of water, preferably sitting upright or standing, to prevent esophageal irritation. Can be taken with food to reduce stomach upset, but avoid dairy products, antacids, and iron supplements within 2 hours of dose.
Mechanism of Action
Inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit, preventing the attachment of aminoacyl tRNA to the ribosomal A site.
Pharmacokinetics
Onset
Onset of antibacterial action within hours.
Excretion
Primarily excreted unchanged in feces (via bile) and urine.
Half life
16-24 hours.
Absorption
Well absorbed orally (nearly 100%), even with food, though dairy products may decrease absorption.
Metabolism
Minimally metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to doxycycline or other tetracyclines
- Pregnancy (especially second and third trimesters)
- Children under 8 years of age (due to potential for tooth discoloration and bone growth inhibition)
Drug Interactions
Warfarin
May enhance anticoagulant effect, requiring INR monitoring.
Penicillins
May interfere with the bactericidal action of penicillins.
Oral contraceptives
May reduce the efficacy of oral contraceptives.
Antacids, iron supplements, calcium supplements, dairy products
May decrease doxycycline absorption.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms may include nausea, vomiting, and diarrhea. Management is supportive; gastric lavage or induced emesis may be considered soon after ingestion. Hemodialysis is not effective.
Pregnancy & Lactation
Pregnancy Category D. Contraindicated during the second and third trimesters due to potential for permanent tooth discoloration and skeletal effects in the fetus. Not recommended during lactation as it is excreted into breast milk and can cause tooth discoloration in infants.
