Description
Overview of the medicine
Doxycycline is a broad-spectrum tetracycline antibiotic used to treat various bacterial infections, including respiratory tract infections, urinary tract infections, skin infections, and certain sexually transmitted infections. It is also used for malaria prophylaxis and acne.
Uses & Indications
Dosage
Adults
Typically 100 mg once or twice daily, depending on the infection. For severe infections, initial dose 200 mg, then 100 mg daily.
Elderly
No specific dosage adjustment is generally needed, but monitor renal and hepatic function, especially with prolonged use.
Renal_impairment
No significant dosage adjustment is usually required as doxycycline is primarily excreted non-renally.
How to Take
Take orally with a full glass of water, preferably during a meal to reduce esophageal irritation. Do not lie down for at least 30-60 minutes after taking the medicine.
Mechanism of Action
Doxycycline works by inhibiting bacterial protein synthesis. It binds reversibly to the 30S ribosomal subunit of susceptible bacteria, preventing the attachment of aminoacyl tRNA to the acceptor site on the mRNA-ribosome complex, thereby inhibiting peptide chain elongation.

Onset
Antibacterial effects typically begin within a few hours of administration.
Excretion
Primarily excreted unchanged in the feces via biliary excretion, with a smaller amount eliminated in urine.
Half life
Approximately 12-22 hours (average 18 hours), allowing for once or twice daily dosing.
Absorption
Well absorbed orally, nearly completely absorbed from the gastrointestinal tract.
Metabolism
Partially metabolized in the liver, primarily via glucuronidation.
Side Effects
Contraindications

Drug Interactions
Penicillins
Tetracyclines can interfere with the bactericidal action of penicillins. Avoid concurrent use.
Methotrexate
Concurrent use may increase methotrexate toxicity.
Oral contraceptives
Doxycycline may reduce the effectiveness of oral contraceptives, leading to breakthrough bleeding or unintended pregnancy. Advise alternative birth control methods.
Anticoagulants (e.g., warfarin)
Doxycycline may potentiate the effects of anticoagulants, requiring dosage adjustment.
Antacids containing aluminum, calcium, or magnesium; iron preparations; and bismuth subsalicylate
These can impair the absorption of doxycycline. Administer doxycycline 2-3 hours before or after these agents.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
In cases of overdose, gastric lavage and symptomatic supportive care are recommended. Doxycycline is not readily removed by hemodialysis.
Pregnancy & Lactation
Pregnancy: Doxycycline is Pregnancy Category D. It is contraindicated during pregnancy as it can cause permanent tooth discoloration (yellow-grey-brown) and inhibit bone growth in the fetus. Lactation: Doxycycline is excreted in breast milk and is not recommended during breastfeeding due to the risk of tooth discoloration and inhibition of bone growth in the nursing infant.
