Description
Overview of the medicine
Doxycycline is a broad-spectrum tetracycline antibiotic used to treat a wide variety of bacterial infections, including respiratory tract infections, urinary tract infections, skin infections, sexually transmitted infections, and certain parasitic infections like malaria.
Uses & Indications
Dosage
Adults
Standard dose: 100 mg orally twice daily on the first day, followed by 100 mg once daily. For severe infections, 100 mg twice daily may be continued. For malaria prophylaxis: 100 mg once daily.
Elderly
No specific dose adjustment is generally required for elderly patients based on age alone. Dosage should be based on the specific infection and patient condition.
Renal_impairment
Dose adjustment is generally not necessary in patients with renal impairment, as doxycycline is primarily excreted non-renally.
How to Take
Take Doxylin capsule orally with a full glass of water, either with or without food. To minimize the risk of esophageal irritation, remain upright for at least 30 minutes to 1 hour after taking the capsule.
Mechanism of Action
Doxycycline works by inhibiting bacterial protein synthesis. It binds reversibly to the 30S ribosomal subunit of susceptible bacteria, thereby blocking the attachment of aminoacyl tRNA to the acceptor site on the mRNA-ribosome complex. This prevents the addition of new amino acids to the growing peptide chain, stopping bacterial growth and reproduction.
Pharmacokinetics
Onset
Therapeutic effects usually seen within 1-2 days, depending on the infection.
Excretion
Mainly excreted unchanged in the feces via the intestine and, to a lesser extent, in the urine.
Half life
The plasma half-life of doxycycline is approximately 16-22 hours, allowing for once or twice daily dosing.
Absorption
Doxycycline is well absorbed after oral administration, typically 90-100% bioavailability. Peak plasma concentrations are achieved within 2-3 hours.
Metabolism
Minimal hepatic metabolism; primarily excreted as inactive epimers or chelates.
Side Effects
Contraindications

Drug Interactions
Warfarin
Doxycycline may potentiate the anticoagulant effect of warfarin. Increased monitoring of INR is recommended.
Penicillins
Tetracyclines may interfere with the bactericidal action of penicillins. Avoid concomitant use.
Oral contraceptives
May reduce the effectiveness of oral contraceptives. Advise patients to use an additional form of contraception.
Antacids (aluminum, calcium, magnesium) and Iron preparations
May decrease the absorption of doxycycline. Take doxycycline at least 2-3 hours before or after these agents.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, seek immediate medical attention. Symptoms may include nausea, vomiting, diarrhea, and potentially liver or kidney damage. Treatment is primarily supportive and symptomatic, including gastric lavage or induced emesis, and administration of activated charcoal if appropriate.
Pregnancy & Lactation
Doxycycline is contraindicated during pregnancy due to the risk of permanent tooth discoloration and skeletal effects in the fetus. It is also excreted into breast milk and is contraindicated during breastfeeding.
