Description
Overview of the medicine
Drotaverine is an antispasmodic medication used to relieve spasms of the smooth muscles in various conditions, particularly those affecting the gastrointestinal, biliary, and urogenital tracts.
Uses & Indications
Dosage
Adults
The usual dose is 40-80 mg, 2 to 3 times daily. Maximum daily dose should not exceed 240 mg.
Elderly
No specific dose adjustment is generally required, but caution is advised due to potential age-related changes in pharmacokinetics and increased sensitivity to adverse effects.
Renal_impairment
No specific dose adjustment is generally needed, but caution should be exercised in severe cases.
How to Take
Take the tablet orally with or without food, as directed by your doctor. Swallow the tablet whole with a glass of water.
Mechanism of Action
Drotaverine inhibits phosphodiesterase-4 (PDE4), an enzyme specific to smooth muscle cells, leading to increased intracellular cyclic AMP (cAMP) levels. This rise in cAMP activates myosin light chain kinase (MLCK), which in turn causes relaxation of smooth muscle cells.
Pharmacokinetics
Onset
15-30 minutes after oral administration.
Excretion
Mainly excreted as metabolites in urine (about 50%) and feces (about 30%) within 72 hours.
Half life
Approximately 8-10 hours.
Absorption
Rapid and almost complete absorption from the gastrointestinal tract after oral administration.
Metabolism
Extensively metabolized in the liver, primarily by O-demethylation.
Side Effects
Contraindications
- •Hypersensitivity to drotaverine or any component of the formulation.
- •Severe hepatic, renal, or cardiac insufficiency (e.g., severe heart failure).
- •Children under 1 year of age (for some formulations).
- •Lactation (breastfeeding mothers).
Drug Interactions
Levodopa
May decrease the anti-parkinsonian effect of levodopa, potentially leading to increased tremor and rigidity.
Other antispasmodics
Concurrent use with other antispasmodics may result in an additive effect.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptoms may include cardiac rhythm disturbances and conduction disorders. Treatment is symptomatic and supportive. Gastric lavage and general supportive measures should be employed.
Pregnancy & Lactation
Pregnancy: Use with caution during pregnancy, especially during the first trimester, only if clearly needed and under medical supervision. Lactation: Contraindicated during breastfeeding due to lack of sufficient safety data.
