Description
Overview of the medicine
Drotaverine is an antispasmodic medication used to relieve spasms of the smooth muscles of various organs, providing relief from pain caused by such spasms.
Uses & Indications
Dosage
Adults
The usual adult dose is 40-80 mg, 1-3 times daily, or as directed by physician.
Elderly
No specific dose adjustment required, but caution is advised in elderly patients, especially those with cardiovascular issues.
Renal_impairment
No specific dose adjustment required in mild to moderate renal impairment, but caution is advised in severe cases.
How to Take
To be taken orally with water, with or without food. The tablet should be swallowed whole.
Mechanism of Action
Drotaverine selectively inhibits phosphodiesterase-4 (PDE4) enzyme, leading to increased intracellular cAMP. This increase in cAMP relaxes smooth muscles by reducing intracellular calcium levels, thereby alleviating spasms and pain.
Pharmacokinetics
Onset
Within 30-60 minutes.
Excretion
Mainly excreted as metabolites in urine (about 50%) and feces (about 30%).
Half life
Approximately 8-10 hours.
Absorption
Rapid and complete absorption after oral administration. Peak plasma concentration reached within 45-60 minutes.
Metabolism
Extensively metabolized in the liver via O-demethylation.
Side Effects
Contraindications
- Hypersensitivity to drotaverine or any excipients
- Severe hepatic, renal, or cardiac insufficiency
- Second or third degree AV block
- Hypotension
- Children below 6 years of age
Drug Interactions
Levodopa
May reduce the antiparkinsonian effect of levodopa due to its PDE4 inhibitory action. Concomitant administration may lead to rigidity and tremor.
Other antispasmodics
Concomitant use with other antimuscarinic antispasmodics may lead to an additive effect, increasing the risk of side effects. Should be avoided or monitored closely.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include cardiac arrhythmias and conduction disorders, severe hypotension. Treatment is symptomatic and supportive, including gastric lavage and monitoring of vital signs and cardiac function.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed and the potential benefit outweighs the potential risk. Not recommended during lactation without physician's advice as it is unknown if drotaverine is excreted in breast milk.
