Description
Overview of the medicine
Esomeprazole is a proton pump inhibitor that reduces acid secretion in the stomach. It is used to treat various acid-related conditions such as GERD, ulcers, and Zollinger-Ellison Syndrome.
Uses & Indications
Dosage
Adults
Typical dosage is 20-40 mg once daily. For GERD: 20-40 mg once daily for 4-8 weeks. For Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks, maintenance 20 mg once daily.
Elderly
No specific dosage adjustment usually needed, but monitor for renal/hepatic impairment.
Renal_impairment
No dose adjustment required for mild to moderate impairment. For severe impairment, caution is advised.
How to Take
Oral, usually taken 30-60 minutes before a meal. Swallow the tablet whole; do not crush, chew, or divide.
Mechanism of Action
Selectively inhibits the H+/K+-ATPase (proton pump) in gastric parietal cells, thereby blocking the final step of acid production.

Onset
Within 1 hour
Excretion
Mainly via urine (approx. 80%) and feces (approx. 20%).
Half life
Approximately 1.3 hours (plasma), but acid suppression effect lasts much longer.
Absorption
Rapidly absorbed, bioavailability is approximately 64% after the first dose and increases to 89% with repeated doses.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Diazepam
Prolonged half-life of diazepam.
Warfarin
Increased INR and risk of bleeding.
Tacrolimus
Increased tacrolimus levels.
Clopidogrel
Reduced antiplatelet effect.
Methotrexate
Increased methotrexate levels.
Atazanavir/Nelfinavir
Decreased antiretroviral levels, avoid concomitant use.

Store below 30°C, away from moisture and direct sunlight. Keep out of reach of children.

Limited experience. Symptoms might include drowsiness, blurred vision, tachycardia, nausea, sweating. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs the potential risk to the fetus. Esomeprazole is excreted in breast milk; use with caution in lactating mothers.
