Description
Overview of the medicine
Duovas 5 mg Tablet is a combination medicine containing Rosuvastatin (a statin) and Fenofibrate (a fibrate). It is used to lower high levels of cholesterol and triglycerides in the blood, especially in patients with mixed dyslipidemia when monotherapy is not sufficient.
Uses & Indications
Dosage
Adults
One tablet once daily, preferably with a meal.
Elderly
No dosage adjustment typically required, but caution due to increased risk of side effects and impaired renal function. Start with lowest effective dose.
Renal_impairment
Contraindicated in severe renal impairment (CrCl < 30 mL/min). For mild to moderate renal impairment, dose adjustment or careful monitoring may be necessary. Rosuvastatin component requires caution with CrCl < 60 mL/min.
How to Take
Take orally, once daily, with or without food. It is recommended to take with a meal to enhance fenofibrate absorption.
Mechanism of Action
Rosuvastatin inhibits HMG-CoA reductase, an enzyme responsible for cholesterol synthesis in the liver, thereby reducing LDL-C. Fenofibrate activates peroxisome proliferator-activated receptor alpha (PPARα), which increases lipolysis and elimination of triglyceride-rich particles from plasma, and reduces the production of apolipoprotein CIII, leading to reduced triglyceride levels and increased HDL-C.
Pharmacokinetics
Onset
Lipid-lowering effects of Rosuvastatin are usually seen within 1 week, with maximum effect by 2-4 weeks. Fenofibrate's maximum effects on lipids are generally achieved within 2 weeks.
Excretion
Rosuvastatin: Primarily excreted unchanged in feces (90%). Fenofibric acid: Primarily excreted in urine as fenofibric acid and fenofibric acid glucuronide.
Half life
Rosuvastatin: Approximately 19 hours. Fenofibric acid (active metabolite of Fenofibrate): Approximately 20 hours.
Absorption
Rosuvastatin: Absolute bioavailability is approximately 20%. Fenofibrate: Well absorbed from the gastrointestinal tract, especially when taken with food; converted to active fenofibric acid.
Metabolism
Rosuvastatin: Limited metabolism, primarily by CYP2C9 and CYP2C19. Fenofibrate: Rapidly hydrolyzed to the active metabolite fenofibric acid, which is glucuronidated and then excreted.
Side Effects
Contraindications
- •Hypersensitivity to Rosuvastatin, Fenofibrate, or any component of the formulation.
- •Severe renal impairment (CrCl < 30 mL/min).
- •Active liver disease or unexplained persistent elevations of serum transaminases.
- •Pre-existing gallbladder disease.
- •History of photosensitivity or photoallergic reaction during treatment with fibrates or ketoprofen.
- •Unexplained muscle pain (myopathy).
Drug Interactions
Colchicine
Increased risk of myopathy/rhabdomyolysis.
Gemfibrozil
Increased risk of myopathy/rhabdomyolysis, avoid co-administration with Fenofibrate.
Cyclosporine
Increased Rosuvastatin exposure, avoid co-administration.
Other fibrates
Avoid combination due to increased risk of muscle toxicity.
Anticoagulants (e.g., Warfarin)
Potentiation of anticoagulant effect, increased risk of bleeding. Close monitoring of INR is required.
CYP3A4 inhibitors (e.g., Itraconazole, Ketoconazole)
May increase statin levels, use with caution.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
No specific antidote. In case of overdose, symptomatic and supportive measures should be taken. Monitor liver function and CK levels. Hemodialysis is unlikely to be beneficial.
Pregnancy & Lactation
Contraindicated during pregnancy and breastfeeding. It may cause fetal harm and is excreted in breast milk.
