Description
Overview of the medicine
Dyana is a combined oral contraceptive pill containing two active ingredients: dienogest (a progestogen) and ethinylestradiol (an estrogen). It is primarily used to prevent pregnancy. In some cases, it may also be used to treat moderate acne and excessive hair growth (hirsutism) in women who desire contraception.
Uses & Indications
Dosage
Adults
One tablet daily for 21 consecutive days, followed by a 7-day tablet-free interval. A new pack is started after the 7-day interval.
Elderly
Not indicated for postmenopausal women.
Renal_impairment
No specific dose adjustment required for mild to moderate renal impairment, but caution is advised. Not recommended for severe renal impairment.
How to Take
Take one tablet orally at approximately the same time each day. Tablets must be taken in the order directed on the blister pack.
Mechanism of Action
Dyana prevents pregnancy primarily by inhibiting ovulation (release of an egg from the ovary). It also causes changes in the cervical mucus, making it more difficult for sperm to reach the uterus, and alters the lining of the uterus, making it less receptive to implantation. The anti-androgenic effects of dienogest contribute to its use in treating acne and hirsutism.
Pharmacokinetics
Onset
Contraceptive effect usually starts after 7 consecutive days of active pill taking, provided it is started on the first day of menstruation.
Excretion
Metabolites of both components are primarily excreted via urine and bile. Dienogest is mainly excreted renally, while ethinylestradiol metabolites are excreted via urine and feces.
Half life
Ethinylestradiol: approximately 12-25 hours. Dienogest: approximately 9-10 hours.
Absorption
Both dienogest and ethinylestradiol are rapidly and almost completely absorbed after oral administration. Peak plasma concentrations are reached within 1-2 hours for both components.
Metabolism
Both are extensively metabolized in the liver, primarily via hydroxylation and glucuronidation. Ethinylestradiol undergoes significant first-pass metabolism.

May decrease contraceptive efficacy by increasing hepatic metabolism of hormones.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms may include nausea, vomiting, and withdrawal bleeding. No specific antidote; treatment is symptomatic and supportive.
Pregnancy & Lactation
Contraindicated during pregnancy. Not recommended during breastfeeding as it may reduce milk production and pass into breast milk.
