Overview of the medicine
Dysmen 250 mg tablet contains Mefenamic Acid, a non-steroidal anti-inflammatory drug (NSAID) used to relieve mild to moderate pain, fever, and inflammation. It is commonly prescribed for menstrual pain (dysmenorrhea) and other acute pain conditions.
Uses & Indications
Dosage
Adults
For acute pain and dysmenorrhea: 500 mg initial dose, then 250 mg every 6 hours as needed. Max 7 days duration. For menorrhagia: 500 mg initial dose, then 250 mg every 6 hours, starting with onset of bleeding and associated symptoms. Max 5 days per cycle.
Elderly
Use with caution; lower doses may be required due to increased risk of adverse effects, especially GI bleeding and renal impairment.
Renal_impairment
Not recommended for patients with severe renal impairment. In moderate impairment, reduce dose and monitor renal function.
How to Take
Take orally with food or milk to minimize gastrointestinal upset. Do not crush or chew the tablet.
Mechanism of Action
Mefenamic Acid inhibits cyclooxygenase (COX-1 and COX-2) enzymes, thereby reducing the synthesis of prostaglandins, which are mediators of pain, inflammation, and fever.

Pain relief typically begins within 30-60 minutes.
Excretion
Primarily excreted in the urine (about 50%) and feces (about 25%) as metabolites.
Half life
Approximately 2-4 hours.
Absorption
Rapidly and well absorbed from the gastrointestinal tract. Peak plasma concentrations are reached in 2-4 hours.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 CYP2C9, to hydroxylated and carboxylated metabolites.
Side Effects
Contraindications

Drug Interactions
Lithium
Increased plasma lithium levels.
Methotrexate

Diuretics/ACE inhibitors
Reduced hypotensive effect and increased risk of renal impairment.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include lethargy, drowsiness, nausea, vomiting, epigastric pain. Rare: gastrointestinal bleeding, acute renal failure, coma, convulsions. Management is symptomatic and supportive; gastric lavage, activated charcoal may be considered within an hour of ingestion.
Pregnancy & Lactation
Pregnancy: Avoid in the third trimester due to potential fetal harm (premature closure of ductus arteriosus). Use with caution in first and second trimesters only if clearly needed. Lactation: Mefenamic Acid is excreted in breast milk; use with caution or consider alternatives.
