Description
Overview of the medicine
Dyvon 0.005% w/w Ointment contains Calcipotriol, a synthetic vitamin D3 derivative. It is primarily used for the topical treatment of mild to moderate plaque psoriasis by regulating skin cell growth and differentiation.
Uses & Indications
Dosage
Adults
Apply to the affected area once or twice daily. Maximum weekly dose should not exceed 100 g of ointment.
Elderly
Same as adult dosage. Exercise caution due to potential increased skin fragility.
Renal_impairment
No specific dosage adjustment is required, but use with caution due to systemic absorption and metabolism.
How to Take
For external use only. Apply a thin layer to the affected skin area and rub in gently and completely. Wash hands after application. Avoid contact with face, eyes, and mucous membranes.
Mechanism of Action
Calcipotriol is a vitamin D analogue that works by binding to vitamin D receptors in skin cells (keratinocytes). This binding modulates the proliferation and differentiation of keratinocytes, slowing down the excessive cell growth characteristic of psoriasis and promoting normal cell development.
Pharmacokinetics
Onset
Clinical improvement usually observed within 2 weeks, with maximum effect in 6-8 weeks.
Excretion
Metabolites are excreted primarily via bile (feces) and to a lesser extent via urine.
Half life
Not well-defined for topical application due to low systemic absorption; systemic half-life of calcipotriol is approximately 2-5 hours after IV administration.
Absorption
Minimal systemic absorption (approx. 1-5%) occurs after topical application, with less than 1% of the applied dose entering the systemic circulation when applied to psoriatic lesions.
Metabolism
Systemically absorbed calcipotriol is rapidly metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- Patients with known hypersensitivity to calcipotriol or any of the excipients
- Patients with known disorders of calcium metabolism
- Not for use on the face, groin, or axillae
- Not for use in erythrodermic, exfoliative, and pustular psoriasis
Drug Interactions
Calcium supplements
Use with caution due to potential increase in serum calcium levels.
Other Vitamin D analogues
Avoid concomitant use as it may increase the risk of hypercalcemia.
Storage
Store below 25°C. Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose include hypercalcemia (high calcium levels in blood). Treatment involves discontinuation of the medication and temporary cessation of calcium supplementation. Serum calcium levels should be monitored.
Pregnancy & Lactation
Pregnancy: Use only if the potential benefit outweighs the potential risk to the fetus. Lactation: It is not known whether calcipotriol is excreted in human milk. Exercise caution when administering to a nursing mother.

Clinical Trials
Numerous clinical trials have established the efficacy and safety of topical calcipotriol for psoriasis treatment, demonstrating significant reduction in plaque severity compared to placebo.
Lab Monitoring
- Serum calcium levels (especially with extensive use or in patients predisposed to hypercalcemia)
Doctor Notes
- Advise patients on proper application technique and maximum weekly dose to minimize systemic absorption and potential hypercalcemia.
- Monitor for signs of skin irritation or worsening of psoriasis.
- Consider combination therapy for severe cases or those unresponsive to monotherapy.
Patient Guidelines
- Apply only to affected areas.
- Do not exceed the recommended weekly maximum dose.
- Avoid contact with eyes, face, and mucous membranes.
- Wash hands thoroughly after use.
- Inform your doctor if your condition worsens or does not improve within 2-4 weeks.
Missed Dose Advice
If you miss a dose, apply it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not apply a double dose to make up for a missed one.
