E-Cof Plus 100 mg Syrup is a combination medication used to relieve symptoms of cough, cold, and flu. It works by loosening mucus, suppressing cough, and clearing nasal congestion.
Uses & Indications
Dosage
Adults
10-20 ml (2-4 teaspoonfuls) every 4-6 hours, not exceeding 6 doses in 24 hours.
Elderly
No specific dose adjustment is usually required, but caution is advised due to potential increased sensitivity.
Renal_impairment
Use with caution; dose reduction may be necessary if severe impairment.
How to Take
Oral. Shake the bottle well before use. Use a measuring spoon/cup for accurate dosage.
Mechanism of Action
Guaifenesin works as an expectorant, reducing the viscosity of mucus in the airways, making it easier to cough up. Dextromethorphan HBr acts as an antitussive, suppressing the cough reflex by direct action on the cough center in the medulla. Phenylephrine HCl is a decongestant that stimulates alpha-adrenergic receptors in the respiratory tract, causing vasoconstriction and reduction of mucosal edema.

Onset
Onset of action typically within 15-30 minutes for decongestion and cough suppression, and expectorant effect within 30 minutes to an hour.
Excretion
Primarily excreted via the kidneys as metabolites and unchanged drug.
Half life
Guaifenesin: approx. 1 hour. Dextromethorphan: 1.4-3.9 hours. Phenylephrine: 2-3 hours.
Absorption
Guaifenesin is rapidly absorbed from the GI tract. Dextromethorphan is readily absorbed from the GI tract. Phenylephrine is well absorbed orally but undergoes significant first-pass metabolism.
Metabolism
Guaifenesin is metabolized via oxidation. Dextromethorphan is extensively metabolized in the liver by O-demethylation (CYP2D6). Phenylephrine is metabolized in the gut and liver by monoamine oxidase.

Contraindications
- Hypersensitivity to any component.
- Patients on MAO inhibitors or within 14 days of stopping them.
- Severe hypertension or coronary artery disease.
- Children under 2 years of age.

Beta-blockers
May potentiate the pressor effects of phenylephrine.
MAO Inhibitors
Concurrent use can lead to hypertensive crisis due to phenylephrine.
CNS Depressants (e.g., alcohol, sedatives)
May enhance CNS depression due to dextromethorphan.
