Description
Overview of the medicine
Ebasten-FT 10 mg Tablet is an orally disintegrating antihistamine medication used to alleviate symptoms of allergic conditions such as seasonal and perennial allergic rhinitis and chronic idiopathic urticaria. The 'FT' (Fast Melt) formulation allows for quick dissolution in the mouth without water, facilitating easier administration.
Uses & Indications
Dosage
Adults
10 mg once daily. In severe cases or for chronic idiopathic urticaria, the dose may be increased to 20 mg once daily.
Elderly
No specific dosage adjustment is required for healthy elderly patients. However, caution is advised in elderly patients with hepatic or renal impairment.
Renal_impairment
No dosage adjustment is necessary for patients with mild to moderate renal impairment. For severe renal impairment, consult a physician.
How to Take
Place the Ebasten-FT tablet on the tongue. It will rapidly disperse and can be swallowed immediately with or without water. It can be taken with or without food, but taking it with food may slightly delay absorption.
Mechanism of Action
Ebastine is a selective, long-acting H1-receptor antagonist. It competitively inhibits the binding of histamine to peripheral H1 receptors, thereby preventing histamine-mediated allergic reactions like sneezing, itching, and watery eyes. Ebastine is rapidly metabolized to its active metabolite, carebastine, which is primarily responsible for its antihistaminic effect.
Pharmacokinetics
Onset
Antihistaminic effect typically begins within 1 hour.
Excretion
Approximately 60-70% of the dose is excreted via urine and 30% via feces, mainly as metabolites.
Half life
The elimination half-life of carebastine is approximately 15-19 hours.
Absorption
Rapidly absorbed after oral administration, undergoes extensive first-pass metabolism to carebastine. Peak plasma levels of carebastine are reached within 2-6 hours.
Metabolism
Extensively metabolized in the liver, primarily by the cytochrome P450 3A4 (CYP3A4) enzyme system, to its active metabolite carebastine.
Side Effects
Contraindications
- •Hypersensitivity to ebastine or any of the excipients of the formulation.
- •Children under 12 years of age (for 10 mg tablet, specific age recommendations may vary by product/country).
- •Severe hepatic impairment.
Drug Interactions
Rifampicin
Concomitant administration with rifampicin, a potent CYP3A4 inducer, may decrease plasma concentrations of ebastine and carebastine, potentially reducing its efficacy.
Ketoconazole, Itraconazole, Erythromycin, Clarithromycin
Concomitant use with these CYP3A4 inhibitors may significantly increase plasma levels of ebastine and its active metabolite carebastine, potentially increasing the risk of QT interval prolongation.
Storage
Store in a cool, dry place below 30°C, protected from light and moisture. Keep out of the sight and reach of children.
Overdose
In cases of overdose, symptoms may include increased drowsiness, central nervous system (CNS) depression, or cardiac effects such as QT prolongation. Management involves symptomatic and supportive treatment. There is no specific antidote. Gastric lavage and activated charcoal may be considered in recent ingestions. Monitor vital signs and ECG.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown teratogenic effects, but human data are limited. Ebasten-FT should be used during pregnancy only if clearly needed and the potential benefits outweigh the potential risks. Not recommended during lactation as ebastine or its active metabolite may be excreted in breast milk.
