Overview of the medicine
Ebazam (Clobazam) is a benzodiazepine derivative primarily used as an anxiolytic and anticonvulsant. It is effective in managing anxiety, certain types of epilepsy, and as an adjunctive treatment for seizures associated with Lennox-Gastaut syndrome.
Uses & Indications
Dosage
Adults
Initial dose: 5 mg once or twice daily. May be slowly increased as needed to 20-30 mg daily, given in divided doses.
Elderly
Start with a lower dose (e.g., 5 mg once daily) and titrate slowly, monitoring closely for adverse effects.
Renal_impairment
No dosage adjustment may be needed in mild to moderate renal impairment. Use with caution in severe cases and consider lower doses.
How to Take
Administer orally, with or without food. To minimize sedation, a larger proportion of the daily dose may be taken at bedtime.
Mechanism of Action
Clobazam selectively binds to benzodiazepine receptors on the GABA-A receptor complex in the central nervous system, enhancing the inhibitory effects of GABA. This leads to hyperpolarization of neurons and reduced neuronal excitability, producing anxiolytic, sedative, anticonvulsant, and muscle relaxant effects.

Onset
Within 30-60 minutes.
Excretion
Excreted mainly by the kidneys (approximately 80%) and via feces (approximately 11%).
Half life
Clobazam: Approximately 18 hours. Active metabolite (N-desmethylclobazam): 36-42 hours.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract. Peak plasma concentrations are reached within 0.5-4 hours.
Metabolism
Primarily metabolized in the liver by CYP2C19 to its major active metabolite, N-desmethylclobazam.
Side Effects
Contraindications

Drug Interactions
Opioids
Increased risk of respiratory depression, profound sedation, and coma.

Other CNS Depressants (e.g., alcohol, barbiturates)
Enhanced CNS depression.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, confusion, lethargy, ataxia, respiratory depression, hypotension, and coma. Management involves supportive care, maintaining airway, and consideration of flumazenil if appropriate.
Pregnancy & Lactation
Clobazam is classified as Pregnancy Category C. There is evidence of risk to the fetus, especially during the first trimester. Excreted in breast milk, so use during lactation is generally not recommended.
