Description
Overview of the medicine
Ecena 100 mg tablet contains Ecetraline, a selective serotonin reuptake inhibitor (SSRI) used to treat major depressive disorder, anxiety disorders, and panic disorder. It works by increasing the levels of serotonin in the brain, which helps to improve mood and reduce anxiety.
Uses & Indications
Dosage
Adults
For Major Depressive Disorder: Initial dose is 50 mg once daily. May be increased to 100 mg after several weeks, up to a maximum of 200 mg daily based on response and tolerability.
Elderly
A lower initial dose (e.g., 25 mg daily) may be considered, with careful titration due to potential increased sensitivity to side effects.
Renal_impairment
No dosage adjustment is generally required for patients with mild to moderate renal impairment. Use with caution in severe renal impairment.
How to Take
Ecena 100 mg tablet should be taken orally once daily, in the morning or evening, with or without food. Swallow the tablet whole with a glass of water, do not chew or crush.
Mechanism of Action
Ecetraline selectively inhibits the reuptake of serotonin (5-HT) by the presynaptic neuron in the central nervous system (CNS). This leads to an increased concentration of serotonin in the synaptic cleft, enhancing serotonergic neurotransmission and ultimately improving mood and reducing anxiety.
Pharmacokinetics
Onset
Therapeutic effects are usually observed after 2-4 weeks of consistent treatment, though some patients may experience initial improvements earlier.
Excretion
Excreted primarily in urine (approximately 40%) and feces (approximately 50%), mainly as metabolites.
Half life
Approximately 26-32 hours for Ecetraline and its major active metabolite, N-desmethyl-Ecetraline.
Absorption
Well absorbed after oral administration, peak plasma concentrations reached in 4-8 hours. Food may slightly increase absorption.
Metabolism
Extensively metabolized in the liver, primarily via CYP2C19 and CYP2B6, to an active metabolite (N-desmethyl-Ecetraline) and other inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to Ecetraline or any component of the formulation.
- •Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing MAOI treatment.
- •Concomitant use with Pimozide.
- •Uncontrolled narrow-angle glaucoma.
Drug Interactions
MAOIs
Concurrent use may lead to serious, sometimes fatal, serotonin syndrome. Avoid.
Alcohol
May potentiate the sedative effects of alcohol; concurrent use is not recommended.
Pimozide
Increased plasma levels of Pimozide, which may lead to QT prolongation and cardiac arrhythmias. Avoid.
NSAIDs, Aspirin, Warfarin
Increased risk of bleeding, particularly gastrointestinal bleeding.
Other Serotonergic Drugs (e.g., Triptans, Tramadol, Linezolid, St. John's Wort)
Increased risk of serotonin syndrome.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, nausea, vomiting, tachycardia, tremor, agitation, dizziness, and rarely, serotonin syndrome. Treatment is symptomatic and supportive; maintain an open airway, monitor cardiac and vital signs. Activated charcoal may be considered. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C/D. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Neonates exposed to SSRIs late in the third trimester have developed complications requiring prolonged hospitalization. Ecetraline is excreted into breast milk; caution should be exercised when administered to a nursing mother. Consult a doctor.
