Description
Overview of the medicine
Edysta 5 mg tablet contains Amlodipine Besilate, a calcium channel blocker used to treat high blood pressure (hypertension) and chest pain (angina). It works by relaxing blood vessels, making it easier for the heart to pump blood.
Uses & Indications
Dosage
Adults
Initial dose 5 mg once daily, maximum 10 mg once daily. Dosage should be adjusted according to individual patient response and tolerance.
Elderly
Usually initiate with 2.5 mg once daily; dosage should be gradually increased as needed and tolerated.
Renal_impairment
No dosage adjustment is required for patients with renal impairment.
Hepatic_impairment
Initial dose 2.5 mg once daily. Use with caution.
How to Take
Take orally, once daily, with or without food. Swallow the tablet whole with a glass of water. It is recommended to take the medicine at the same time each day.
Mechanism of Action
Amlodipine is a dihydropyridine calcium antagonist (calcium ion antagonist or slow channel blocker) that inhibits the transmembrane influx of calcium ions into cardiac and vascular smooth muscle. This leads to peripheral arterial vasodilation and reduction in afterload, as well as coronary vasodilation.
Pharmacokinetics
Onset
Within 1-2 hours for antihypertensive effect.
Excretion
Mainly excreted in the urine (60% as metabolites, 10% as unchanged drug), and 20-25% via feces.
Half life
30-50 hours, allowing once-daily dosing.
Absorption
Well absorbed after oral administration with peak plasma concentrations reached between 6-12 hours. Bioavailability is 64-90%.
Metabolism
Extensively metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to amlodipine or other dihydropyridine derivatives, or to any of the excipients.
- •Severe hypotension
- •Cardiogenic shock
- •Obstructive outflow tract disease of the left ventricle (e.g., severe aortic stenosis)
- •Hemodynamically unstable heart failure after acute myocardial infarction
Drug Interactions
Simvastatin
Limit simvastatin dose to 20 mg daily due to increased simvastatin exposure.
Clarithromycin
Increased risk of hypotension and kidney injury.
CYP3A4 inducers (e.g., rifampicin)
May decrease amlodipine exposure, monitor blood pressure.
CYP3A4 inhibitors (e.g., ketoconazole, erythromycin)
May increase amlodipine exposure, consider dose reduction.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Overdose may result in excessive peripheral vasodilation with marked hypotension and possibly reflex tachycardia. Symptomatic hypotension may require active cardiovascular support, including elevation of extremities, fluid administration, and vasopressors if appropriate. Calcium gluconate may be beneficial to reverse calcium channel blocking effects.
Pregnancy & Lactation
Pregnancy Category C. Amlodipine should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether amlodipine is excreted in human milk. Exercise caution when administered to a nursing woman.
