Effical-400 mg tablet is a combination supplement containing Calcium Carbonate and Vitamin D3, primarily used to treat and prevent calcium and vitamin D deficiencies. It supports bone health, density, and strength.
Uses & Indications
Dosage
Adults
Typically, 1-2 tablets daily, or as directed by the physician. It should be taken with meals to enhance absorption.
Elderly
Same as adult dosage, but caution for renal function.
Renal_impairment
Use with caution and dose adjustment may be required. Regular monitoring of serum calcium and phosphate levels is advisable.
How to Take
Take orally with a glass of water, preferably with meals. Do not crush or chew the tablet unless it's a chewable formulation.
Mechanism of Action
Calcium is an essential mineral required for bone formation and maintenance, muscle contraction, nerve transmission, and blood clotting. Vitamin D3 (Cholecalciferol) aids in the absorption of calcium from the gut and its deposition in bones, regulating calcium and phosphate homeostasis.
Pharmacokinetics
Onset
Clinical effects on bone density are gradual, over weeks to months.
Excretion
Mainly via feces (unabsorbed calcium), and urine (excess absorbed calcium). Vitamin D3 metabolites are excreted primarily in bile and feces.
Half life
Calcium: Approximately 20-30 hours for elimination from plasma. Vitamin D3: 15-30 days.
Absorption
Calcium carbonate is absorbed in the small intestine, with absorption enhanced by Vitamin D3. Approximately 20-30% of dietary calcium is absorbed.
Metabolism
Calcium is not metabolized. Vitamin D3 is hydroxylated in the liver to 25-hydroxyvitamin D [25(OH)D] and then in the kidneys to the active form, 1,25-dihydroxyvitamin D [1,25(OH)2D].
Side Effects
Contraindications
- •Hypercalcaemia (high calcium levels in blood)
- •Hypercalciuria (high calcium levels in urine)
- •Severe renal impairment
- •Known hypersensitivity to calcium carbonate, Vitamin D3, or any component of the formulation
Drug Interactions
Levothyroxine
Calcium can interfere with levothyroxine absorption. Separate doses by at least 4 hours.
Bisphosphonates
Calcium can interfere with bisphosphonate absorption. Administer at least 30-60 minutes before calcium.
Thiazide diuretics
May increase the risk of hypercalcaemia due to decreased urinary calcium excretion.
Tetracycline and fluoroquinolone antibiotics
Calcium can decrease the absorption of these antibiotics. Administer at least 2 hours before or 4-6 hours after calcium.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of hypercalcaemia include nausea, vomiting, constipation, thirst, polyuria, anorexia, muscle weakness, and fatigue. Severe cases can lead to cardiac arrhythmias and coma. Treatment involves discontinuation of the supplement, hydration, and medical management of hypercalcaemia.
Pregnancy & Lactation
Generally considered safe and beneficial during pregnancy and lactation for meeting increased calcium and vitamin D requirements, but should be used under medical supervision.
