Description
Overview of the medicine
Efirel 10 mg Tablet contains Efirel, a lipid-lowering medication of the statin class. It is primarily used to reduce high cholesterol levels and triglycerides in the blood, thereby lowering the risk of heart attack, stroke, and other heart-related conditions.
Uses & Indications
Dosage
Adults
Initial dose is usually 10 mg once daily. The dose can be adjusted by the physician based on lipid levels and therapeutic response, up to a maximum of 80 mg once daily.
Elderly
No specific dose adjustment is required for elderly patients. However, caution should be exercised, and dose titration should be done carefully.
Renal_impairment
No dose adjustment is necessary for patients with mild to moderate renal impairment. For severe renal impairment, consultation with a physician is recommended.
How to Take
Efirel 10 mg Tablet should be taken orally, once daily. It can be taken any time of the day, with or without food. It is recommended to take it at the same time each day to maintain consistent drug levels.
Mechanism of Action
Efirel selectively inhibits HMG-CoA reductase, an enzyme responsible for cholesterol synthesis in the liver. This inhibition leads to a decrease in hepatic cholesterol, which stimulates the upregulation of LDL receptors on the hepatocyte surface, enhancing the clearance of LDL from the blood.
Pharmacokinetics
Onset
Significant lipid-lowering effects typically observed within 2 weeks; maximum effect within 4 weeks.
Excretion
Primarily excreted in the bile and feces. Less than 2% is excreted in the urine.
Half life
Approximately 14 hours for Efirel, but 20-30 hours for active metabolites, contributing to prolonged activity.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Extensively metabolized in the liver by cytochrome P450 3A4 (CYP3A4) to active and inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Efirel or any component of the tablet.
- Active liver disease or unexplained persistent elevations of serum transaminases.
- Pregnancy and lactation.
- Concomitant use with strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin).
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin; monitor INR closely.
Cyclosporine
Significant increase in Efirel exposure; dose adjustment may be necessary.
Gemfibrozil, other Fibrates
Increased risk of myopathy/rhabdomyolysis when co-administered.
Strong CYP3A4 Inhibitors (e.g., Itraconazole, Ritonavir, Clarithromycin)
Can significantly increase Efirel plasma concentrations, increasing risk of myopathy.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
There is no specific treatment for overdose with Efirel. In the event of an overdose, patients should receive symptomatic and supportive treatment as required. Liver function tests and CK levels should be monitored.
Pregnancy & Lactation
Efirel is contraindicated during pregnancy and breastfeeding. Women of childbearing potential should use effective contraception during treatment. If pregnancy occurs, Efirel should be discontinued immediately.
