Ofloxacin is a broad-spectrum fluoroquinolone antibiotic used to treat a variety of bacterial infections.
Uses & Indications
Dosage
Adults
Typical dose is 250 mg twice daily for 7-14 days, depending on the type and severity of infection. For uncomplicated UTIs, 250 mg once daily for 3 days.
Elderly
No specific dose adjustment is needed based on age alone, but renal function should be assessed, and dosage adjusted accordingly if impaired.
Renal_impairment
Dosage adjustment is necessary. For creatinine clearance 20-50 mL/min, administer the usual dose every 24 hours. For creatinine clearance <20 mL/min, administer half the usual dose every 24 hours.
How to Take
Efroxin-250-mg-tablet should be taken orally with water. It can be taken with or without food. Patients should drink plenty of fluids to prevent crystalluria.
Mechanism of Action
Ofloxacin works by inhibiting bacterial DNA gyrase and topoisomerase IV, enzymes crucial for DNA replication, transcription, repair, and recombination, leading to bacterial cell death.

Clinical effects typically within 1-2 hours.
Excretion
Primarily excreted unchanged in the urine (70-80% within 24-48 hours).
Half life
Approximately 4-7 hours, which may be prolonged in renal impairment.
Absorption
Well absorbed orally, with good bioavailability (approximately 90-100%). Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Minimally metabolized in the liver (less than 10% of the dose).
Side Effects
Contraindications
- •Hypersensitivity to Ofloxacin or other quinolone antibiotics.
- •History of tendon disorders related to quinolone administration.
- •Patients with epilepsy or other seizure disorders.
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin, requiring INR monitoring and dose adjustment.
Corticosteroids
Increased risk of tendon rupture.
Antacids, Iron/Zinc Salts, Sucralfate
May reduce the absorption of Ofloxacin. Administer Ofloxacin 2 hours before or 2 hours after these agents.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May increase the risk of CNS stimulation and convulsive seizures.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include central nervous system effects (e.g., confusion, seizures), gastrointestinal reactions (e.g., nausea, vomiting), and prolongation of the QT interval. Management is symptomatic and supportive, including gastric lavage and adequate hydration.
Pregnancy & Lactation
Pregnancy Category C. Ofloxacin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in breast milk and should not be used during breastfeeding due to potential risk of arthropathy in the infant.
