Overview of the medicine
Doxycycline is a broad-spectrum antibiotic used to treat various bacterial infections, including respiratory tract infections, urinary tract infections, skin infections, and certain sexually transmitted diseases.
Uses & Indications
Dosage
Adults
Standard dose: 100 mg orally once or twice daily. For severe infections, 100 mg every 12 hours. For acne: 50-100 mg daily.
Elderly
No specific dose adjustment required based on age alone, but renal/hepatic function should be considered for co-morbidities.
Renal_impairment
No dose adjustment is usually needed for patients with renal impairment due to major non-renal excretion pathway.
How to Take
Take with a full glass of water (at least 240 mL), preferably while sitting or standing, to prevent esophageal irritation and ulceration. Do not lie down for at least 30 minutes after taking the capsule. Can be taken with food to minimize gastric upset, but avoid dairy products and antacids.
Mechanism of Action
Doxycycline inhibits bacterial protein synthesis by reversibly binding to the 30S ribosomal subunit, thereby preventing the attachment of aminoacyl tRNA to the A site on the ribosome.

Contraindications

Drug Interactions
Warfarin
Doxycycline may potentiate the anticoagulant effect of warfarin. Monitor INR closely.
Penicillins
Tetracyclines can interfere with the bactericidal action of penicillins. Avoid concomitant use.
Oral contraceptives
May reduce the effectiveness of oral contraceptives. Advise alternative or additional birth control methods.
Barbiturates, Carbamazepine, Phenytoin
May decrease the half-life of doxycycline, potentially reducing its effectiveness.
Antacids, Iron supplements, Bismuth subsalicylate
These can decrease the absorption of doxycycline. Separate administration by at least 2-3 hours.

Symptoms of overdose may include nausea, vomiting, and diarrhea. Management involves gastric lavage and symptomatic and supportive treatment. Dialysis is generally not effective in removing doxycycline from the body.
