Overview of the medicine
Elipta 25 mg Tablet contains Sertraline, an antidepressant used to treat major depressive disorder, obsessive-compulsive disorder, panic disorder, and social anxiety disorder, among others.
Uses & Indications
Dosage
Adults
Initial: 25-50 mg once daily. May be increased gradually by 25-50 mg at weekly intervals to a maximum of 200 mg daily.
Elderly
Similar to adults, but starting with a lower dose (e.g., 25 mg) and slower titration may be considered due to potential for decreased clearance.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. Caution is advised in severe renal impairment.
How to Take
Take orally once daily, with or without food. It is generally recommended to take it at the same time each day.
Mechanism of Action
Sertraline selectively inhibits the reuptake of serotonin (5-HT) by presynaptic neurons, leading to an increase in the concentration of serotonin in the synaptic cleft, thereby enhancing serotonergic neurotransmission.
Pharmacokinetics
Onset
Therapeutic effects typically begin within 2 to 4 weeks, with full effect potentially taking longer.
Excretion
Approximately 40-45% excreted in urine and 40-45% in feces, primarily as metabolites.
Half life
Approximately 26 hours for sertraline, and 62 to 104 hours for its active metabolite, N-desmethylsertraline.
Absorption
Well absorbed orally; peak plasma concentrations achieved 4.5 to 8.4 hours post-dose. Bioavailability is approximately 44%.
Metabolism
Extensively metabolized in the liver via multiple cytochrome P450 enzymes (CYP2D6, CYP2C19, CYP3A4, CYP2B6, CYP2C9) to N-desmethylsertraline, which is less active.
Side Effects
Contraindications
- Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing MAOI therapy (risk of serotonin syndrome).
- Concomitant use with pimozide.
- Known hypersensitivity to sertraline or any component of the formulation.
- Concomitant use with disulfiram (for oral solution formulation due to alcohol content).

Risk of serious, sometimes fatal, serotonin syndrome.
Lithium
Potential for increased tremor.
Pimozide
Increased pimozide levels and potential for QT prolongation.
Triptans

Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Sertraline is excreted in breast milk; use with caution in nursing mothers.
