Description
Overview of the medicine
Elisa 3 mg Tablet contains Eszopiclone, a non-benzodiazepine hypnotic, used for the short-term treatment of insomnia. It helps individuals fall asleep faster and stay asleep longer.
Uses & Indications
Dosage
Adults
The recommended starting dose is 1 mg. The dose can be increased to 2 mg or 3 mg if clinically indicated. Should be taken immediately before bedtime. Maximum recommended dose is 3 mg.
Elderly
For elderly or debilitated patients, the recommended starting dose is 1 mg immediately before bedtime. Dosage should not exceed 2 mg.
Renal_impairment
No dose adjustment is required for mild to moderate renal impairment. Use with caution in severe renal impairment (starting dose 1 mg).
How to Take
Administer orally immediately before bedtime. Take with or without food. Avoid taking with a heavy, high-fat meal as it may delay absorption.
Mechanism of Action
Eszopiclone is a non-benzodiazepine hypnotic that is a pyrrolopyrazine derivative of the cyclopyrrolone class. It is thought to act by interacting with GABA-A receptor complexes at binding domains close to or allosterically coupled to benzodiazepine receptors, thereby enhancing the inhibitory effects of GABA.
Pharmacokinetics
Onset
Within 30-60 minutes.
Excretion
Excreted primarily in urine (up to 75%) and feces (20-25%) as metabolites.
Half life
Approximately 6 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are reached within approximately 1 hour.
Metabolism
Extensively metabolized by oxidation and demethylation via CYP3A4 and CYP2E1 enzymes.
Side Effects
Contraindications
- Hypersensitivity to eszopiclone or any component of the formulation.
- Patients with severe hepatic impairment.
- Concomitant use with strong CYP3A4 inhibitors (unless dose adjusted).
- Patients with complex sleep behaviors (e.g., sleepwalking, sleep-driving) after taking eszopiclone.
Drug Interactions
Alcohol
Additive CNS depressant effects.
Other CNS depressants (e.g., benzodiazepines, opioids)
Increased risk of sedation and respiratory depression.
Strong CYP3A4 inducers (e.g., rifampin, carbamazepine)
Can decrease eszopiclone exposure, reducing efficacy.
Strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin)
Can significantly increase eszopiclone exposure, requiring dose reduction.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, confusion, ataxia, slurred speech, and respiratory depression. Management includes general supportive measures, gastric lavage if appropriate, and monitoring of vital signs. Flumazenil may be considered in severe cases, but its use is not routinely recommended.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs the potential risk to the fetus. Eszopiclone is excreted in breast milk; therefore, use with caution in nursing mothers.
