Description
Overview of the medicine
em-pill 30 mg tablet is an emergency contraceptive medicine containing Mifepristone, used to prevent pregnancy after unprotected sex or contraceptive failure. It works by delaying or inhibiting ovulation and preventing implantation in the uterus.
Uses & Indications
Dosage
Adults
One 30 mg tablet orally as soon as possible, preferably within 72 hours, but can be taken up to 120 hours (5 days) after unprotected intercourse.
Elderly
Not typically indicated for elderly women as emergency contraception is for reproductive age. No specific dosage adjustment required for those of reproductive age.
Renal_impairment
No specific dosage adjustment recommended, but use with caution in severe renal impairment.
How to Take
Take the tablet orally with water. It can be taken with or without food. The sooner it is taken after unprotected intercourse, the more effective it is.
Mechanism of Action
Mifepristone acts as an antiprogestin by binding to progesterone receptors, blocking the effects of progesterone. This can delay or inhibit ovulation, alter the endometrium to prevent implantation, and interfere with follicle rupture.
Pharmacokinetics
Onset
Effects on ovulation and endometrium typically begin within hours of administration.
Excretion
Mainly excreted in feces (approximately 80%) and urine (approximately 9%) as metabolites.
Half life
Terminal half-life ranges from 12 to 72 hours, influenced by enterohepatic recirculation.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4, into active and inactive metabolites.
Side Effects
Contraindications
- •Known or suspected pregnancy
- •Hypersensitivity to mifepristone or any component
- •Undiagnosed vaginal bleeding
Drug Interactions
Corticosteroids
Mifepristone has antiglucocorticoid activity, potentially reducing corticosteroid efficacy.
CYP3A4 inducers (e.g., Rifampicin, Phenytoin)
May decrease mifepristone levels, reducing efficacy.
CYP3A4 inhibitors (e.g., Ketoconazole, Erythromycin)
May increase mifepristone levels.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and may include nausea, vomiting, diarrhea, and headache. No specific antidote. Management is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy: Contraindicated in established pregnancy. Lactation: Small amounts may pass into breast milk. A temporary interruption of breastfeeding for 3-4 days after taking the tablet can be considered.
