Overview of the medicine
Emedox 20 mg Tablet contains Esomeprazole, a proton pump inhibitor used to reduce stomach acid production. It is commonly prescribed for conditions like gastroesophageal reflux disease (GERD), stomach ulcers, and other conditions involving excessive stomach acid.
Uses & Indications
Dosage
Adults
GERD: 20 mg or 40 mg once daily for 4-8 weeks. Erosive Esophagitis: 20 mg or 40 mg once daily for 4-8 weeks. Zollinger-Ellison Syndrome: Initial 40 mg twice daily, adjusted as needed.
Elderly
No specific dosage adjustment is generally required for elderly patients, but caution is advised due to potential age-related decrease in renal or hepatic function.
Renal_impairment
No dosage adjustment is needed for patients with renal impairment.
Hepatic_impairment
For severe hepatic impairment (Child-Pugh Class C), a maximum dose of 20 mg once daily should not be exceeded.
How to Take
Take the tablet whole with a glass of water, at least one hour before a meal. Do not crush or chew the tablet.
Mechanism of Action
Esomeprazole works by irreversibly blocking the proton pump (H+/K+-ATPase) in the parietal cells of the stomach lining. This action reduces the secretion of gastric acid, providing relief from acid-related symptoms and allowing healing of the esophagus and stomach.

Onset
Within 1 hour, full effect achieved within 2-3 days of daily dosing.
Excretion
Mainly excreted in urine (approximately 80%) and feces (approximately 20%) as inactive metabolites.
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma levels reached within 1-2 hours.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system (primarily CYP2C19 and CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to Esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Diazepam
May decrease the clearance of diazepam.
Warfarin
Increased INR and prothrombin time, monitor closely.
Clopidogrel
Esomeprazole may reduce the antiplatelet effect of clopidogrel.
Methotrexate
May increase methotrexate levels, requiring dose adjustment.
Drugs dependent on gastric pH for absorption (e.g., ketoconazole, iron salts)
Reduced absorption.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

Symptoms of overdose may include drowsiness, tachycardia, and increased stomach acid secretion. Management is symptomatic and supportive. There is no specific antidote.
