Description
Overview of the medicine
Emestop 40 mg Capsule contains Omeprazole, a proton pump inhibitor used to reduce stomach acid production. It is used to treat conditions such as heartburn, acid reflux, ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
GERD: 20-40 mg once daily for 4-8 weeks. Ulcers: 20 mg once daily for 4-8 weeks. Zollinger-Ellison Syndrome: Initial 60 mg once daily, adjusted as needed.
Elderly
No specific dosage adjustment is required, but caution is advised in very frail patients.
Renal_impairment
No dosage adjustment necessary.
How to Take
Take the capsule whole with water at least 30 minutes before a meal, preferably in the morning. Do not chew or crush.
Mechanism of Action
Omeprazole irreversibly blocks the H+/K+-ATPase enzyme system (the 'proton pump') in the gastric parietal cells, thereby inhibiting the final step in acid production.
Pharmacokinetics
Onset
Within 1 hour; maximal effect within 2 hours.
Excretion
Mainly excreted in urine (approx. 80%) and feces (approx. 20%) as metabolites.
Half life
Approximately 0.5-1 hour.
Absorption
Rapidly absorbed from the gastrointestinal tract, bioavailability is about 30-40% due to first-pass metabolism.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4 enzymes.
Side Effects
Contraindications
- Hypersensitivity to omeprazole or other substituted benzimidazoles
- Concomitant use with nelfinavir
Drug Interactions
Digoxin
Increased digoxin absorption due to decreased gastric acidity.
Warfarin
Increased INR and prothrombin time. Monitor closely.
Clopidogrel
May reduce the antiplatelet effect of clopidogrel. Consider alternative antiplatelet therapy or a different PPI.
Methotrexate
May increase methotrexate levels and toxicity, especially with high doses.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.

Symptoms of overdose include nausea, vomiting, dizziness, abdominal pain, diarrhea, and headache. Treatment is symptomatic and supportive.
