Overview of the medicine
Emfogen-M XR 10 mg Tablet is a combination medicine containing Empagliflozin and Metformin (extended-release), used to improve glycemic control in adults with type 2 diabetes mellitus. It helps to lower blood sugar levels, and may also reduce the risk of cardiovascular events and hospitalization for heart failure in certain patients.
Uses & Indications
Dosage
Adults
Initially, one tablet (Empagliflozin 10 mg/Metformin XR) once daily, usually with the evening meal. Dose may be titrated based on efficacy and tolerability. Maximum recommended daily dose of Empagliflozin is 25 mg.
Elderly
Use with caution. Assess renal function frequently. Dosage adjustment may be required based on renal function.
Renal_impairment
Not recommended for eGFR < 30 mL/min/1.73 m². Dose adjustment required for eGFR between 30-60 mL/min/1.73 m² (e.g., lower Metformin dose or avoid if eGFR < 45 for Metformin).
How to Take
Take orally once daily with a meal, preferably the evening meal, to minimize gastrointestinal side effects associated with Metformin. Swallow the tablet whole; do not crush, cut, or chew.
Mechanism of Action
Empagliflozin inhibits sodium-glucose co-transporter 2 (SGLT2) in the kidneys, leading to increased urinary glucose excretion. Metformin reduces hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity by increasing peripheral glucose uptake and utilization.

Onset
Empagliflozin: Glycosuric effect seen within 30 min, peak effect in hours. Metformin: Onset of action is gradual, with full effect taking several days to weeks.
Excretion
Empagliflozin: Excreted via urine (approx. 50%) and feces (approx. 50%). Metformin: Excreted unchanged in the urine.
Half life
Empagliflozin: Approximately 10-13 hours. Metformin: Approximately 6.2 hours.
Absorption
Empagliflozin is rapidly absorbed orally; Metformin XR has prolonged absorption.
Metabolism
Empagliflozin is primarily metabolized by glucuronidation. Metformin is not metabolized.
Side Effects
Contraindications
- •Hypersensitivity to empagliflozin, metformin, or any excipients.
- •Severe renal impairment (eGFR < 30 mL/min/1.73 m²).
- •Metabolic acidosis, including diabetic ketoacidosis, with or without coma.
- •Acute or chronic metabolic acidosis, including lactic acidosis.
- •Severe dehydration, sepsis, acute heart failure, or other conditions that increase the risk of lactic acidosis.
- •Known history of lactic acidosis.
Drug Interactions
Alcohol
Increases the risk of lactic acidosis with metformin.
Loop Diuretics
May increase the risk of dehydration and hypotension, and potentially renal impairment.
Radiocontrast Agents
Temporarily discontinue metformin at the time of or prior to iodinated contrast imaging procedures and for 48 hours afterward.
Insulin and Insulin Secretagogues
Increased risk of hypoglycemia when co-administered; dose adjustments may be needed.
Carbonic Anhydrase Inhibitors (e.g., topiramate, acetazolamide)
May increase the risk of lactic acidosis with metformin.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose of empagliflozin can lead to hypoglycemia (especially with concomitant use of insulin/sulfonylureas) and fluid loss. Metformin overdose can lead to lactic acidosis. Treatment involves symptomatic and supportive measures, gastric lavage, and hemodialysis for severe metformin overdose.
Pregnancy & Lactation
Not recommended during pregnancy, especially during the second and third trimesters due to potential renal effects on the fetus (Empagliflozin). Metformin can be used during pregnancy if benefits outweigh risks, but consultation with a doctor is essential. Not recommended during breastfeeding.
