Overview of the medicine
Emitof 4 mg Tablet contains Ondansetron, an antiemetic medication used to prevent nausea and vomiting caused by cancer chemotherapy, radiation therapy, and surgery. It works by blocking serotonin, a natural substance in the body that can cause nausea and vomiting.
Uses & Indications
Dosage
Adults
Highly emetogenic chemotherapy: 24 mg orally once daily, 30 minutes before chemotherapy. Moderately emetogenic chemotherapy: 8 mg orally twice daily. Post-operative nausea/vomiting: 16 mg orally one hour before anesthesia.
Elderly
No specific dosage adjustment is generally required. For patients over 75 years, initial IV dose should not exceed 16 mg.
Renal_impairment
No specific dosage adjustment is required for patients with renal impairment.
How to Take
Take the tablet orally with or without food, as directed by your doctor. Do not crush, chew, or break the tablet unless specifically advised.
Mechanism of Action
Ondansetron is a selective 5-HT3 receptor antagonist. Chemotherapeutic agents and radiotherapy may cause release of serotonin (5-HT) in the small intestine, which activates vagal afferents via 5-HT3 receptors to initiate the vomiting reflex. Ondansetron blocks the initiation of this reflex.

About 5% excreted unchanged in urine, remaining as metabolites in urine and feces.

Approximately 3-5.5 hours
Absorption
Rapidly and completely absorbed from the GI tract after oral administration; subject to some first-pass metabolism.
Metabolism
Extensively metabolized in the liver by hydroxylation, followed by glucuronide or sulfate conjugation. CYP1A2, CYP2D6, and CYP3A4 are involved.
Side Effects
Contraindications
- •Hypersensitivity to Ondansetron or any component of the formulation.
- •Concomitant use with apomorphine (due to risk of profound hypotension and loss of consciousness).
Drug Interactions
Tramadol
May reduce analgesic effect of tramadol; risk of serotonin syndrome.
Apomorphine
Concomitant use contraindicated due to risk of severe hypotension and loss of consciousness.
SSRIs/SNRIs
Increased risk of serotonin syndrome.
QT-prolonging drugs
Increased risk of QT interval prolongation.
Phenytoin, Carbamazepine, Rifampicin
May reduce ondansetron levels.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include transient visual disturbances, severe constipation, hypotension, and vasovagal episodes with transient AV block. Treatment is symptomatic and supportive. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. It is not known whether Ondansetron is excreted in human milk. Exercise caution when administered to a nursing woman.
