Overview of the medicine
Empagliflozin is an oral antidiabetic drug used to improve glycemic control in adults with type 2 diabetes mellitus. It also reduces the risk of cardiovascular death in patients with type 2 diabetes and established cardiovascular disease, and is indicated for heart failure and chronic kidney disease.
Uses & Indications
Dosage
Adults
Initial dose is 10 mg once daily in the morning, with or without food. May be increased to 25 mg once daily if tolerated and additional glycemic control is needed.
Elderly
No specific dose adjustment needed based on age. Renal function should be considered.
Renal_impairment
Not recommended for initiation in patients with eGFR < 30 mL/min/1.73 m². Dose adjustment may be needed based on eGFR.
How to Take
Take orally once daily in the morning, with or without food.
Mechanism of Action
Empagliflozin selectively inhibits SGLT2 in the renal proximal tubules, reducing glucose reabsorption and increasing urinary glucose excretion. This leads to reduced blood glucose levels.

Mainly excreted via urine (approximately 50%) and feces (approximately 50%).

Side Effects
Contraindications
- •Hypersensitivity to empagliflozin or any component of the formulation.
- •Patients with severe renal impairment (eGFR < 30 mL/min/1.73 m²), end-stage renal disease, or on dialysis.
Drug Interactions
Diuretics
Increased risk of dehydration and hypotension.
UGT Inducers (e.g., Rifampicin)
May reduce empagliflozin exposure.
Insulin or Insulin Secretagogues
Increased risk of hypoglycemia (dose adjustment may be required).
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose may lead to symptoms such as dehydration, electrolyte imbalance, and hypoglycemia. Management includes supportive treatment, monitoring of vital signs and electrolytes, and correction of hypoglycemia if present.
Pregnancy & Lactation
Not recommended during the second and third trimesters of pregnancy due to potential adverse renal effects on the fetus. Advise against breastfeeding due to potential risk to the infant.
