Description
Overview of the medicine
Empadus 25 mg Tablet contains Empagliflozin, an SGLT2 inhibitor used for the treatment of type 2 diabetes mellitus, heart failure, and chronic kidney disease.
Uses & Indications
Dosage
Adults
For Type 2 Diabetes: Initial dose is 10 mg once daily, taken in the morning, with or without food. May be increased to 25 mg once daily if needed. For Heart Failure/CKD: 10 mg once daily.
Elderly
No dose adjustment generally required based on age alone. Monitor renal function.
Renal_impairment
No dose adjustment needed for mild to moderate renal impairment. Not recommended for eGFR < 20 mL/min/1.73 m².
How to Take
Take orally once daily in the morning, with or without food.
Mechanism of Action
Empagliflozin inhibits sodium-glucose co-transporter 2 (SGLT2) in the kidneys, reducing glucose reabsorption from the glomerular filtrate and lowering the renal threshold for glucose. This leads to increased urinary glucose excretion, which lowers plasma glucose concentrations.
Pharmacokinetics
Onset
Within 30 minutes (glucose lowering effect).
Excretion
Primarily excreted via the urine (approx. 50%) and feces (approx. 50%).
Half life
Approximately 12 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations reached in 1.5 hours.
Metabolism
Extensively metabolized by glucuronidation.
Side Effects
Contraindications
- Hypersensitivity to empagliflozin or any excipients.
- Severe renal impairment (eGFR < 20 mL/min/1.73 m²), end-stage renal disease, or dialysis.
Drug Interactions
Lithium
SGLT2 inhibitors may decrease serum lithium concentrations.
Diuretics
May increase the risk of dehydration and hypotension.
Insulin and Insulin Secretagogues (e.g., Sulfonylureas)
Increased risk of hypoglycemia, may require dose reduction of insulin/sulfonylurea.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In the event of an overdose, symptomatic and supportive treatment should be initiated. Empagliflozin is not removed by dialysis.
Pregnancy & Lactation
Not recommended during pregnancy, especially in the second and third trimesters. It is unknown if empagliflozin is excreted in human milk; use is not recommended during breastfeeding.

Clinical Trials
Empagliflozin has been extensively studied in large-scale clinical trials (e.g., EMPA-REG OUTCOME, EMPEROR-Reduced, EMPEROR-Preserved, EMPA-KIDNEY) demonstrating its efficacy and cardiovascular/renal benefits.
Lab Monitoring
- Renal function (e.g., eGFR) before initiation and periodically thereafter.
- Blood glucose levels.
- Lipid profile (periodically).
Doctor Notes
- Advise patients about the importance of hydration to reduce the risk of volume depletion.
- Educate patients on symptoms of DKA and genital mycotic infections.
- Regularly monitor renal function and electrolytes.
Patient Guidelines
- Take exactly as prescribed by your doctor.
- Stay well-hydrated.
- Report any signs of genital infection, UTI, or unusual symptoms to your doctor.
- Do not stop taking the medicine without consulting your doctor.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular schedule. Do not take a double dose.
Driving Precautions
Empagliflozin generally does not impair the ability to drive or operate machinery. However, if used in combination with insulin or sulfonylureas, the risk of hypoglycemia may increase, potentially affecting driving ability.
Lifestyle Advice
- Follow a balanced diet and engage in regular physical activity as advised by your healthcare provider.
- Monitor blood sugar levels regularly.
- Maintain good hygiene to reduce the risk of infections.
