Description
Overview of the medicine
Empoli 10 mg Tablet contains Empagliflozin, an SGLT2 inhibitor, used to improve glycemic control in adults with type 2 diabetes mellitus. It also reduces the risk of cardiovascular death and hospitalization for heart failure in certain patient populations.
Uses & Indications
Dosage
Adults
Initial dose is 10 mg once daily, taken in the morning, with or without food. May be increased to 25 mg once daily based on efficacy and tolerability.
Elderly
No dosage adjustment is required based on age alone, but renal function should be assessed before initiation and monitored periodically.
Renal_impairment
For glycemic control, initiation is not recommended if eGFR is <30 mL/min/1.73m². For heart failure or chronic kidney disease indications, empagliflozin can be initiated at 10 mg once daily down to an eGFR of 20 mL/min/1.73m².
How to Take
Empoli Tablet should be taken orally once daily in the morning, with or without food. Swallow the tablet whole with water.
Mechanism of Action
Empagliflozin works by inhibiting the sodium-glucose co-transporter 2 (SGLT2) in the kidneys, leading to decreased reabsorption of glucose from the glomerular filtrate and increased urinary glucose excretion. This lowers blood glucose levels independently of insulin.
Pharmacokinetics
Onset
Glycemic effects typically observed within weeks of treatment initiation.
Excretion
Approximately 50% renally excreted and 50% fecally excreted, as unchanged drug and metabolites.
Half life
Approximately 12.4 hours.
Absorption
Rapidly absorbed, peak plasma concentration within 1.5 hours. High oral bioavailability.
Metabolism
Primarily metabolized via glucuronidation by UGTs (UGT1A3, UGT1A8, UGT1A9, UGT2B7).
Side Effects
Contraindications
- Known hypersensitivity to empagliflozin or any of its excipients.
- Patients with severe renal impairment (eGFR <30 mL/min/1.73m²) when used for glycemic control.
- End-stage renal disease or patients on dialysis.
Drug Interactions
Lithium
Empagliflozin may decrease serum lithium concentrations, leading to reduced efficacy of lithium. Monitor lithium levels carefully.
Diuretics
Co-administration with diuretics may increase the risk of dehydration and hypotension.
Insulin and Insulin Secretagogues (e.g., Sulfonylureas)
Increased risk of hypoglycemia; a lower dose of insulin or sulfonylurea may be required.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, no specific antidote is known. Treatment should be supportive and based on the patient's clinical presentation. Empagliflozin is unlikely to be removed by dialysis.
Pregnancy & Lactation
Not recommended during the second and third trimesters of pregnancy. Empagliflozin is not recommended during breastfeeding due to potential risk to the infant.

WHO Essential Medicine
YesClinical Trials
Empagliflozin has been extensively studied in several large-scale clinical trials, including EMPA-REG OUTCOME, EMPEROR-Reduced, EMPEROR-Preserved, and EMPA-KIDNEY, which demonstrated its efficacy in glycemic control and its cardiovascular and renal benefits.
Lab Monitoring
- Renal function (eGFR) before initiation and periodically thereafter.
- Blood glucose levels regularly.
- Electrolytes (e.g., sodium, potassium) if risk of dehydration.
- Lipid profile (Empagliflozin can sometimes slightly increase LDL-C).
Doctor Notes
- Counsel patients on hydration and symptoms of genitourinary infections.
- Monitor renal function before and during treatment.
- Be vigilant for signs of DKA, even in patients with only moderately elevated blood glucose.
- Adjust concomitant insulin/sulfonylurea doses to mitigate hypoglycemia risk.
Patient Guidelines
- Take the medicine exactly as prescribed by your doctor.
- Drink plenty of fluids to avoid dehydration.
- Monitor for symptoms of urinary tract or genital infections and seek medical attention if they occur.
- Be aware of symptoms of ketoacidosis (nausea, vomiting, abdominal pain, excessive thirst, rapid breathing) and seek urgent medical help if experienced.
- Inform your doctor about all other medications you are taking.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Empagliflozin itself does not directly impair driving ability, but it can cause dizziness or hypoglycemia (especially when combined with insulin or sulfonylureas). Patients should be cautious if experiencing these effects.
Lifestyle Advice
- Maintain a balanced diet and regular exercise as advised by your healthcare provider.
- Regularly monitor your blood glucose levels.
- Avoid excessive alcohol consumption.
- Stay well-hydrated, especially in warm weather or during illness.
